Objective: Discussing relation of red blood cell distribution width(RDW), NT-pro BNP and chronic obstructive pulmonary disease with pulmonary hypertension.And provide some clinical guidance of condition assessment for pulmonary hypertension with chronic obstructtive disease pulmonary.Methods: Using retrospective analysis methods, collect 306 COPD patients from Respiratory in Gansu Provincial People's Hospital from September 2013 to July 2015,including 116 cases of females and 190 cases of male with a median age of 69.57 ± 10.55 years; The COPD group was divided into pure COPD group(66 cases), COPD and pulmonary hypertension group(240 cases) in basis of pulmonary artery pressure 36 mm Hg measured by echocardiography; Pulmonary hypertension and COPD group was further grouped into: COPD and mild PH group ?(36mm Hg PASP <50mm Hg), COPD and moderate PH group(50mm H?g PASP <75mm H?g), COPD and severe PH group(PASP 75 mm Hg) according to the level of pulmonary artery pressure, which were collected 81 cases, 104 cases, 55 cases. And collect 50 healthy people in medical center of the same period, including 27 cases of males and 23 cases of female with a median age of 68.40 ± 10.59 years. Collect all subjects of age, gender, past history, RDW, NT-pro BNP and other testing indicators. And use SPSS 21.0 statistical software to process data and analyze, and compare levels of RDW and NT-pro BNP between groups, as well as correlate of RDW, NT-pro BNP and pulmonary artery pressure, and P <0.05 was considered statistically significance.Results:1.RDW differences were statistically significant by comparing Healthy control group and COPD group(T=27.36, P < 0.05); RDW differences were statistically significant by comparing Healthy control group, pure COPD group and COPD with pulmonary hypertension group(F=48.21,P<0.05); RDW differences were statistically significant by comparing COPD and mild PH group, COPD and moderate PH group, COPD and severe PH group(F=23.18,P<0.05).2. NT-pro BNP differences were statistically significant by comparing Healthy control group and COPD group(T=18.40,P<0.05); NT-pro BNP differences were statistically significant by comparing Healthy control group, pure COPD group and COPD with pulmonary hypertension group(F=6.99,P<0.05); NT-pro BNP differences were statistically significant by comparing COPD and mild PH group, COPD and moderate PH group, COPD and severe PH group(F=7.76,P<0.05).3.In COPD group, pulmonary artery pressure and red blood cell distribution width or NT-pro BNP were positively correlated(r=0.497, P<0.01; r=0.346, P<0.01).4. In COPD group, red blood cell distribution width and NT-pro BNP was positively correlated(r=0.415, P<0.01).Conclusion:1. AS levels of RDW and NT-pro BNP in patients of COPD with pulmonary hypertension group were higher than pure COPD patients, they can be used as one of the clinical indicators of COPD with pulmonary hypertension to assess the disease.2.AS levels of RDW and NT-pro BNP in patients of COPD and severe PH group were higher than COPD and mild PH group or COPD and moderate PH group, they can be used as one of the clinical indicators of COPD with pulmonary hypertension to assess the disease of severity.
