Abstract/Summary:
Objective:To investigate the correlation between red blood cell distribution width and fibrinogen and the severity of chronic obstructive pulmonary disease(COPD).Methods:1 A total of 298 patients with COPD between January 2017 and May 2018 of the Second Hospital of Shanxi Medical University were selected as the study group,including 199 patients with acute exacerbation as the AE group,and 99 patients with stable phase as the stable group.99 cases served as the control group;2 Record age,gender,GOLD rating,RDW,FIB value and smoking status;3 Compare the differences in RDW and FIB values between the study group and the control group,the AE group and the stable group;4 The research group divided them into four levels of Ⅰ,Ⅱ,Ⅲ,and Ⅳ based on the GOLD classification,and plotted the ROC curve.The area under the curve was used to compare the correlation between the three indicators of RDW,FIB,and FDW + FIB(FDW + FIB The value is an index that combines RDW and FIB by principal component analysis),and Z-test is used to verify the difference between different AUCs.Results:1 There were no significant differences in age,gender,and smoking status between the study group and the control group(P> 0.05);2 The smoking patients in the study group had higher RDW values than the non-smokers,and the same was true in the control group.Smoking in the study group and the control group had no effect on the FIB value;3 The RDW value in the study group was higher than the RDW value in the control group,P <0.05;the FIB value in the study group was higher than the control group FIB value,P <0.05;4 RDW and FIB values in the AE group were higher than those in the stable group(P < 0.05);5 RDW values are ranked at each level Ⅰ<Ⅱ<Ⅲ<Ⅳ;Ⅰ FIB3.47±0.57,ⅡFIB3.96±0.59,Ⅲ FIB5.15 ± 0.57,Ⅳ FIB17.01 ± 1.22,FIB values are ranked at each levelⅠ<Ⅱ<Ⅲ<Ⅳ.6 The AUC values of RDW,FIB,and RDW + FIB are all greater than 0.7,all with certain accuracy.After Z-test processing,it is concluded that the correlation between the three indicators and level RDW = RDW + FIB>FIB,level II FIB>RDW + FIB >RDW,level III FIB = RDW + FIB > RDW,level IV three indicators There was no difference,and the combined detection of RDW and FIB could predict the severity of COPD.Conclusion:1 The RDW value of smokers is higher than that of non-smokers;2 RDW and FIB can be used as biomarkers to predict the acute exacerbation of COPD;3 RDW and FIB are positively correlated with the severity of COPD.The accuracy of the combined detection of COPD severity is better than a single indicator.
