Objective The morbidity and mortality of chronic obstructive pulmonary disease(COPD)is increasing gradually,which has caused great harm to human health in the world.Red cell distribution width(RDW)in the blood is a simple routine detection index.To explore the Clinical significance of red blood cell distribution width(RDW)in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods In this study,we collected the clinical data of 80 hospitalized patients with AECOPD in Fuyang People’s Hospital from October 2015 to May 2016,and the follow-up data of the patients with COPD and the clinical data of 50 patients with in our hospital.The difference in datas of 80 AECOPD patients,42 cases of outpatient follow-up for patients with COPD and 50 cases of healthy control group,were compared with RDW,red blood cell count(RBC),hemoglobin(Hb).AECOPD patients according to the lung function GOLD classification were divided into GOLD II,GOLD III,GOLD IV,and then compared to the three groups of RDW,RBC and Hb.AECOPD patients according to whether with carbon dioxide(CO2)retention,pulmonary artery hypertension were divided into group A(neither CO2 retention and no pulmonary hypertension),group B(pure CO2retention),group C(simple pulmonary hypertension),group D(with CO2 retention,pulmonary arteryhigh pressure)and compared with four groups of RDW levels.The correlation was analyzed with RDW between total hospitalization time and total hospitalization expenses of AECOPD patients.Results1.Baseline characteristics of patients with AECOPD,80 patients with acute exacerbation of chronic obstructive pulmonary disease(n = 70%)were included in this study.Among them,there were male(n = 56)and female(n = 30%).In the case of RDW elevation or not,RDW was higher than 15.5%,and RDW was higher in the RDW group,and RDW was less than 15.5% in the non elevated group.The male and female patients,smoking,FEV1/ expected value in the RDW group and the nonelevated group was statistically significant(P<0.05),no statistical significance in age,hemoglobin,red blood cell count in two groups(P>0.05).2.The difference of RDW levels in the AECOPD group,COPD group and healthy control group was statistically significant(P<0.05),but Hb and RBC in the three groups were not statistically significant(P>0.05).The level of RDW in the AECOPD group and COPD group was compared with the healthy control group(P<0.05),and the difference was statistically significant.3.Red cell distribution width in AECOPD according to GOLD classification: GOLD II,GOLD III,GOLD grade three group RDW levels were(12.6 ± 0.7)% and(13.2 ± 1.1)% and(13.6 ±1.3)%.RDW level in GOLD II,GOLD III,and GOLD IV was statistical significance(P<0.05),and RDW levels in the three groups decreased progressively.Differences in the level of RBC and Hb were no significant in three groups(P>0.05).).The levels of RDW in the three groups were grade Ⅳ(13.6±1.3)%,grade Ⅲ(13.2±1.1)%,grade Ⅱ(12.6±0.7)%.4.The difference in the four groups RDW levels was statistically significant in AECOPD patients according to whether the merger of CO2 retention and pulmonary hypertension(P<0.05).The levels of RDW in the four groups were(12.6±0.7)%,(13.1±0.9)%,(13.3±1.1)%,(14.3±1.5)%.5.RDW level was positively correlated with total hospitalization time and total cost of hospitalization.Conclusion Red blood cell distribution width increased serum levels in patients with acute exacerbation of chronic obstructive pulmonary disease with clinical significance.Red blood cell distribution width increased serum levels in patients with acute exacerbation of chronic obstructive pulmonary disease with clinical significance.RDW can be regarded as a severity marker for patients with AECOPD.
