Analysis On The Medication Rule Of Professor Wang Ke-xian In Treating Chronic Cholecystitis Based On Data Mining

Objective: With the help of data mining software,we analyzed Professor Wang Ke-xian’s clinical treatment of chronic cholecystitis through data mining of clinical records,and combined with learning from teachers and oral teaching,in order to summarize Professor Wang Ke-xian’s clinical experience in treating chronic cholecystitis more comprehensively and objectively,refine his diagnostic and therapeutic ideas,and provide a basis for clinical evidence-based treatment and experience summary.Methods: The prescriptions of Professor Wang Ke-xian for the treatment of chronic cholecystitis from January 2021 to December 2022 were collected and collated,and a database of prescriptions and medications was established after errata were made,and the data were mined by various methods such as frequency analysis,cluster analysis and association rule analysis using the TCM heritage aid platform.Results: 1.A total of 110 cases and 202 prescriptions were included in this study.202 prescriptions were analyzed by frequency statistics,involving 5 types of evidence,and the frequency of evidence was 71 times(35.15%)for damp-heat of liver and gallbladder,51 times(25.25%)for disharmony of liver and spleen,41 times(20.30%)for liver depression and qi stagnation,26 times(12.87%)for loss of nourishment of liver ligaments,and 13 times(6.44%)for stasis of blood and ligaments.The pharmacological analysis of 139 herbal medicines showed that there were 1605 times(40.68%)of warm and 1493 times(37.85%)of cold herbal medicines in the treatment of chronic cholecystitis.The frequency of drug taste was mainly bitter and pungent,accounting for 34.70% and 33.09% of the total data,respectively,followed by sweet,accounting for 19.44% of the total data.The order of drug attribution was spleen >stomach > liver > lung > gallbladder > large intestine > heart > kidney > small intestine > bladder > triple jiao > pericardium.2.Among the 202 prescriptions included,139 herbal medicines were used 4234 times in total.The drugs used more than 50% of the time were,in descending order,Chai Hu,roasted licorice,fried Atractylodes,ginger hemorrhiza,scutellaria,Huang Lian,Mu Xiang,dry ginger,fried peony,aromatic herb,large belly bark,perilla leaf,hedgehog,green peel,codonopsis,rhubarb,and yujin.By drug association analysis and confidence analysis,the most commonly used drug combinations were Ginger Panax,Scutellaria,Huang Lian,and Bupleurum.By entropy hierarchical cluster analysis,eight new prescriptions were obtained.Conclusion: Professor Wang Ke-xian proposed the fundamental treatment for chronic cholecystitis on the basis of the academic thought of "keeping the middle,regulating the liver,and harmonizing the five organs" : "If the damp-heat is not clear,the evil toxins will not be removed;if the damp-heat is not detrimental to the bile and liver,the damp-heat will not be eliminated;if the internal organs are not clear,the evil toxins will not be removed",and summarized it as the four major methods for treating chronic cholecystitis: "dredge,benefit,pass,and clear".
