Clinical Experience Study On Professor Wang Xu’s Regular Pattern Of Medication Use In The Treatment Of Thyroid Nodules Based On Data Mining

Objective:Through the clinical study of copying prescriptions with my teacher,collect the effective medical cases of Professor Wang Xu’s treatment of thyroid nodule,apply data mining technology,systematically summarize Professor Wang Xu’s clinical diagnosis and treatment of thyroid nodule syndrome distribution law,pathogenesis characteristics,syndrome differentiation and treatment methods and the core drugs in the prescription,regular medication pairs,extract the core prescription and analyze the drug taste,function,sorting.This paper summarizes Professor Wang Xu’s experience in the treatment of thyroid nodules and the law of medication,so as to provide the diagnosis and treatment ideas for the treatment of this disease with traditional Chinese medicine.Methods:From May 1,2019 to October 1,2019,the medical records of patients with thyroid nodules who first came to the outpatient clinic were collected and sorted out,and the medical records were screened strictly according to the inclusion and exclusion criteria,and the medical records collection and storage data base was established,Using frequency analysis,cluster analysis,association rules and other methods to mine,analyze the rules and correlations among symptoms,pathogenesis,treatment methods and prescriptions in the following medical records,analyze,summarize and summarize the mining results,and finally extract the commonly used treatment methods,drugs,drug groups and core prescriptions of Professor Wang Xu in the treatment of thyroid nodules.Result:A total of 138 medical records were collected in this study.The results of medical record analysis are as follows:43 pathogenesis and 154 drugs were involved in this study.The majority of thyroid nodule patients are female.The main clinical symptoms are neck discomfort,restless sleep at night,irritability,loose stools,chilliness and fatigue.The tongue is mainly red,light red and dark red.The tongue coating is mostly thin white and yellow,and the pulse is mainly thin,smooth,stringy and heavy.The pathogenesis of the disease is mainly qi stagnation and phlegm obstruction,liver stagnation and qi stagnation,qi stagnation and blood stasis,spleen deficiency and dampness,Qi deficiency and phlegm obstruction.The treatment methods include qi circulation,phlegm resolving,liver dispersing,blood activating and spleen invigorating,supplemented by the methods of Qi replenishing,Nourishing Yin,Yang warming and blood nourishing.In treatment,both deficiency and excess are taken into account,the specimen is treated together,and the liver and spleen are in the same harmony.The core drugs include Fritillaria thunbergii,Platycodon grandiflorum,stir fried white peony,Prunella vulgaris,Scutellaria barbata,Chen xiang qu,Gleditsia sinensis,liquorice,tangerine peel,Hedyotis diffusa,Pinellia ternata,cat claw grass.The commonly used medicine pairs are:saponifia-Fritillaria thunbergii,chenxiangqu-stir fried white peony,chenxiangqu-Fritillaria thunbergii,Scutellaria-Fritillaria thunbergii,Prunella-Platycodon,Prunella-Fritillaria thunbergii,stir fried white peony-Fritillaria thunbergii,Platycodon Fritillaria thunbergii.The commonly used medicine groups were(1)Chinese trumpet creeper,plum blossom,light bamboo leaf,Acorus calamus,dandelion;(2)Salvia miltiorrhiza,Eclipta,rose,floating wheat,Baiwei,lily,Yizhiren,Ligustrum lucidum;(3)white fresh skin,cicada exuviate,Schisandra chinensis,Fangfeng;(4)chuanniuxi,Eupatorium,wangbuliuxing,Cyperus;(5)orange leaf,Tangerine seed,and pedicel.The core prescriptions are as follows:Self-heal,Fritillaria thunbergii,pinellia,Scutellaria,Hedyotis diffusa,Gleditsia sinensis,cat claw grass,Stir fried peony root,Chinese bellflower,Dried tangerine peel,Chenxiangqu,liquorice.Conclusion:It was found that:(1)the pathogenesis of thyroid nodule was mainly due to the loss of liver,spleen,phlegm coagulation and blood stasis in front of the neck;the disease location is mainly in the liver and spleen,which is related to the heart,kidney and stomach;the pathological factors are qi stagnation、phlegm coagulation and blood stasis,and most of them have deficiency syndrome.(2)The clinical syndrome differentiation is mainly based on the comprehensive consideration of distinguishing benign and malignant,disease location,constitution,pathological factors and nodular nature,and the comprehensive treatment of regulating qi,resolving phlegm,removing blood stasis and tonifying deficiency.The method of regulating qi is mainly to regulate qi,clear Qi and descending Qi;the method of eliminating phlegm is mainly to regulate Qi,remove dampness,invigorate spleen,clear heat,warm cold phlegm;the method of removing blood stasis is mainly to promote qi and remove blood stasis,activate blood and remove blood stasis,and break blood and remove blood stasis;according to the deviation of yin and Yang of Qi and blood and the emphasis of pathological viscera,it can be roughly divided into four methods:benefiting spleen,nourishing liver and blood,nourishing liver and kidney,warming spleen and kidney。Different treatment methods often complement each other and should be used for each other,and the method of regulating qi should be used throughout the treatment.(3)The basic recipe includes Fritillaria thunbergii,pinellia Scutellaria barbata Hedyotis diffusa,saxijiao thorn,cat claw grass,fried white peony,Platycodon grandiflorum,tangerine peel,chenxiangqu,liquorice.On this basis,the prescriptions are usually added and subtracted according to the multiple syndrome differentiation systems such as disease nature,disease location,constitution and pathological factors,combined with the patients’
