Objective1.The Traumatic and Routine Stressors Scale(TRSS-EN)for Emergency Department Nurses was introduced and tested for reliability among emergency department nurses for applicability to clinical research.2.The Chinese version of the TRSS-EN scale was applied to emergency nurses to describe the frequency of occurrence of routine and traumatic stressors and the current status of their degree of emotional impact, and to investigate their influencing factors.Methods1.After obtaining authorization from the source authors, the Chineseization process of the scale was strictly followed to translate the English version of the TRSS-EN scale and combine it with back translation to form the first draft of the Chineseized scale;group discussion and expert correspondence were used for cross-cultural adaptation, and 30 emergency department nurses were included to conduct a pre-survey on the degree of understanding of the scale and the time required to fill it out,and the problems found in the pre-survey were improved to form the final version of the scale;a Convenience sampling was used to select 292 emergency department nurses from tertiary level A general hospitals in Beijing,Tianjin and Hebei for the formal survey.Item analysis(correlation coefficient method),validity analysis(content validity, structural validity and convergent validity)and reliability analysis(Cronbach’s alpha coefficient and retest reliability)were used to verify the applicability and stability of the Chinese version of the TRSS-EN scale.2.In the second phase,a cross-sectional survey research method was used to select 339 emergency nurses different from the first batch of emergency nurses in the emergency department of a tertiary level A general hospital in Beijing,Tianjin and Hebei.Guided by PTSD and stress response theory, the general information questionnaire,self-efficacy scale, mental toughness scale and coping style questionnaire were selected to investigate the frequency of traumatic and conventional stressors and the current situation of their emotional impact on emergency department nurses and their influencing factors.Results1.Results of scale translation The scale translation included two parts, the frequency of TRSS-EN scale for emergency nurses and the degree of emotional impact of TRSS-EN scale for emergency nurses,in which both subscales had the same dimensions, number of entries, and content.2.Results of Expert Correspondence Eight experts were consulted on this scale, and the expert recommendation rate for the content of the scale entries was 60%.3.Results of item analysis After item analysis, the results showed that the item decision values of the scale, the correlation between the items and the total score of the scale, the correlation between the corrected items and the total score of the scale, the Cronbach’s alpha coefficient at the time of item deletion ≥ the alpha value of the questionnaire, the commonality values of the items, and the factor loadings met each criterion,so that none of the items were deleted.4.Validity analysis results Content validity: The correlation between each item and the study content was assessed, and the results showed that the I-CVI of each item of the scale ranged from 0.805 to 1.000,and the S-CVI/Ave and S-CVI/UA of the scale were 0.938 and0.859,respectively.(2)Structural validity: Exploratory factor analysis was used to measure the structural validity of the two subscales, and the final two subscales The final two subscales extracted 2 common factors, and the commonality of each question item of the scale was >0.400,and the cumulative variance explained by the 2 common factors was greater than60%.(3)The PTSD scale was selected as the extrinsic validity of this study to estimate the validity of the validity association. The results showed that the correlation coefficient of the frequency subscale was 0.571(P < 0.01)and the correlation coefficient of the degree subscale was 0.564(P < 0.01),indicating that the Chinese version of the TRSS-EN had good validity of the scales.5.Results of reliability analysis(1)Internal consistency test: the Cronbach’s alpha coefficient of the TRSS-EN frequency subscale and the Cronbach’s alpha coefficient of the degree of emotional influence subscale in this study was 0.851,and the Cronbach’s alpha coefficient of the degree of emotional influence subscale was 0.845.(2)Split-half reliability:the questionnaire was divided into two parts, and the results of the study showed that The split-half reliability of TRSS-EN frequency subscale was 0.852,and the split-half reliability of the degree subscale was 0.837,indicating good stability of the scale.(3)The correlation coefficients were greater than 0.7,which indicated that the scale had good stability across time.6.Results of the analysis of the status and influencing factors(1)The total score of the TRSS-EN frequency subscale was(65.60±8.56),the frequency score of the occurrence of conventional stressors was(31.30±4.52),and the frequency score of traumatic stressors was(34.53±5.49);the total score of the TRSS-EN degree of emotional impact subscale was(64.85±10.65).The degree of influence of conventional stressors on the mood of emergency nurses was scored as(30.20±5.05),and the degree of influence of traumatic on the mood of emergency nurses was scored as(34.85±6.30).The frequency of traumatic and routine stressors and the degree of impact on mood and scores on each dimension among emergency department nurses were at a moderately high level.(2)Different age, years in emergency medicine, title, number of night shifts, and negative coping styles had an effect on the frequency of traumatic and routine stressors among emergency department nurses(P < 0.05).(3)Different age, education level, marital status, years in emergency medicine, title, number of night shifts,self-efficacy,and negative coping style had an effect on the degree of emotional impact of traumatic and routine stressors in emergency department nurses(P <0.05).Conclusion1.Chinese version of the TRSS-EN scale was formed and showed good reliability and validity in the domestic emergency nurses population.2.The frequency of traumatic and conventional stressors among emergency department nurses was influenced by different age, years in emergency medicine, job title, number of night shifts, and negative coping;the degree of influence of traumatic and conventional stressors on the mood of emergency nurses was influenced by different age, education level, marital status, years in emergency medicine, job title, number of night shifts,self-efficacy,and negative coping. |
