Objective: Gut microbiota(GM)diversity changes were observed in patients with acute gout.In this study,using microbial 16 S r DNA sequencing technology,combined with bioinformatics analysis,to observe the changes of GM in acute gout patients before and after taking Chuanhu anti-gout mixture(CAGM),analyze the effects of CAGM on GM in patients with acute gout,explore new targets for drug therapy,and provide theoretical basis for the clinical treatment of acute gout with Traditional Chinese medicine.Methods: This study used a prospective research method.30 patients in the acute gout group were selected from patients with acute gout who visited the Affiliated Hospital of Qingdao University from January 2020 to December 2021.The healthy control group consisted of 30 persons,selected from the healthy persons who underwent physical examination in the Affiliated Hospital of Qingdao University during the same period.30 patients in the treatment group were acute gout patients who were treated with CAGM for2 weeks.Record the basic information of all personnel,including gender,height,weight and age,and calculate the body mass index(BMI).All three groups detected C-reactive protein(CRP),erythrocyte sedimentation rate(ESR),uric acid(UA),alanine aminotransferase(ALT),aspartate aminotransferase(AST),creatinine(Cr),and collected stool samples for 16 S r DNA Detect and GM analysis.Results:1 Comparison of general data and clinical indicators between acute gout group and healthy control group In the acute gout group,age(44.900 years vs.54.467 years,P<0.05),BMI(26.958Kg/m2 vs.23.699 Kg/m2,P<0.05),UA(516.733 μmol/L vs.312.603 μmol/L,P <0.05),CRP(8.323mg/L vs.3.478 mg/L,P<0.05),ESR(15.490mm/1h vs.10.067mm/1h,P<0.05)compared with the healthy control group,there were significant differences difference..2 Comparison of clinical indicators between treatment group and acute gout groupAfter CAGM treatment,UA(393.300 μmol/L vs.516.733 μmol/L,P<0.05),CRP(6.533 mg/L vs.8.323 mg/L,P<0.05),ESR(12.833 mm/1h vs..15.490 mm/1h,P <0.05)the average level was lower than that of the acute gout group,with significant significance3 Bioinformatics analysisMicrobial alpha diversity and beta diversity analysis revealed differences in biodiversity and richness between acute gout group and healthy control group,treatment group and acute gout group.Wilcoxon analysis of microbial mean richness showed:(1)Compared between the healthy group and the acute gout group:(1)At the phylum level: Firmicutes and Actinobacteria had a high average abundance;while Bacteroidetes,Desulfovibrio and Proteobacteria had a low average abundance.Except for Proteobacteria,which had no significant difference(P>0.05),the rest of the flora had significant differences(P<0.05).(2)At the genus level:Anaerostipes,Peptostreptococcus,Streptomyces,Actinomyces,Fusicatenibacter,Turicibacter and Erysipelatoclostridium had a high average abundance;while Lactobacillus,Bilophila,Bacteroides,Parabacteroides,Prevotella,Sphingomonas,Coprobacter and Ruminococcus had a low average abundance.Except for Bacteroides,Streptomyces and Erysipelatoclostridium,there was no significant difference(P>0.05),the rest of the flora had significant differences(P<0.05).(2)Comparison between the treatment group and the acute gout group:(1)At the phylum level: Proteobacteria had a high average abundance;while Bacteroidota,Spirochaetota and Firmicutes had a low average abundance.All bacterial groups were significantly different(P<0.05).(2)At the genus level: Catellicoccus,Enterorhabdus,Enterobacter,Sphingomonas and Sediminibacterium had a high average abundance;while Streptomyces,Bacteroides,Campylobacter,Prevotella,Pediococcus,Treponema,Lachnospira and Lachnoclostridium had a low average.All bacterial groups were significantly different(P<0.05).Conclusion:1 Compared with healthy people,the levels of CRP and ESR increase in acute gout patients with hyperuricemia,and the application of CAGM can reduce UA level and inflammatory indexes CRP and ESR levels in acute gout patients.2 Microbial analysis showed that compared with healthy people,acute gout patients had changes in GM diversity and decreased species richness.3 After taking CAGM,the type and abundance of GM in patients with acute gout can be adjusted.
