Objective:To explore the clinical value of dual source CT in the diagnosis of gouty arthritis,Promote the application of dual source CT uric acid crystal deposition analysis technique in clinical practice.Methods:190 patients were selected from September 2015-March 2017 in the Guangxi Medical University First Affiliated Hospital who were scanned by DSCT dual energy technique and meet the inclusion criteria(Including general clinical data,laboratory tests,imaging studies).Record the above data into the Excel database and introduce SPSS statistical software to analyze after arranging.Results:Comparison of DSCT detection of uric acid crystals:gout group detected the highest rate,suspected gout group followed,non-gout group lowest.The sensitivity of DSCT to gout diagnosis was 93.5%,specificity was 96.8%,positive predictive value was 99.0%,negative predictive value was 81.1%.33 cases of normal urinary acid gout patients in 31 cases of DSCT(+),of which 5 cases in the examination of the Department of joint puncture also found urinate crystallization.There were 35 cases of gout in the suspected gout group,and there was no significant difference in the detection rate of urinary urination between the seizures and the contralateral joints.bone destruction in the gout group is associated with uric acid crystallization volume and gout nodules(respectivelyOR=1.051,P=0.039;OR=10.444,P=0.001).Conclusion:DSCT detection of uric acid crystals of the sensitivity and specificity are high,a scan can be on the uric acid crystallization and bone destruction analysis,which not only help the diagnosis of gout,but also for disease assessment.For patients with normal urinary acid suspicious gout,conditional circumstances can be used dual-source CT to assist in the diagnosis.
