| Objective:This study aims to observe and investigate the clinical effects on treating patients with obesity who are diagnosed with hypertensive nephropathy(CKD3-4)by combining the Huatan Qushi decoction and amlodipine.Methods:A controlled group therapy was conducted in the Outpatient department of the First Affiliated Hospital of Heilongjiang University of Traditional Chinese Medicine.During the process of treatment,data of 60 obese patients who were diagnosed with hypertensive nephropathy(CKD3-4)by western medicine and Phlegm dampness type by traditional Chinese medicine respectively were collected to perform the above controlled group therapy.Moreover,the random number table method was applied to divide the target patients into the controlled and treatment group with 30 cases in each group.During the period of treatment,the Huatan Qushi decoction was assigned to the treatment group whereas the Uremic Clearance Granule to the controlled group and both treatments of the groups were based on the routine treatment and the use of Amlodipine.The course of the entire treatment process is 8 weeks.During this period,patients’ indicator changes in both groups were observed separately on the basis of blood pressure,serum creatinine,blood urea nitrogen,estimated glomerular filtration rate,Serum cystatin C,β2-microglobulin,and 24-hour urine protein quantification for weeks 0,4,and 8.The changes in TCM syndrome scores was also observed together in the 0 and 8th week.Consequently,this method was applied to evaluate the clinical efficacy of the combination of Huatan Qushi decoction and Amlodipine to obese patients with hypertensive nephropathy.Results:1.Our analysis showed that the efficacy of the treatment group was higher than that of the controlled group in the comparison of TCM syndrome scores,and the results of both groups after the treatment period were lower than those in the same group before the treatment(P<0.05).2.The clinical effective rate of the treatment group was(83.3%),which was better than the rate of the controlled croup.which was 60.0%.(P<0.05).3.After 4 weeks treatment,the results of systolic blood pressure,systolic blood pressure and mean arterial pressure in both groups were lower than those in the same group before the treatment,After 8 weeks of treatment,it was lower than that after 4 weeks of treatment(P<0.05、.The effect of the treatment group was better than that of the control group after 4 and 8 weeks,which is statistically sicnificant(P<0.05).4.After 4 weeks treatment,the results of serum creatinine,blood urea nitrogen,serum cystatin C,β2-microglobulin,and 24-hour urine protein quantification in both groups were lower than those in the same group before the treatment.However,the estimated glomerular filtration rate was higher than that of the same group before treatment.After 8 weeks of treatment,except that the estimated glomerular filtration rate was higher than that before treatment in the same group(P<0.05),other indexes were lower than that before treatment in the same croup(P<0.05).Conclusion:1.In summary,the clinical symptoms of treating patients with obesity who are diagnosed as hypertensive nephropathy(CKD3-4)are capable of improving by combining the Huatan Qushi decoction and amlodipine.2.Huatan Qushi decoction combined with amlodipine is better than Niaoduqing granule combined with amlodipine in reducing blood pressure in obese patients with hypertensive nephropathy(CKD3-4)with phlegm dampness excess.3.The effects on obese patients with hypertensive nephropathy(CKD3-4)of reducing serum creatinine,blood urea nitrogen,serum cystatin C,β2-microglobulin,24-hour urine protein quantification and increasing the estimated glomerular filtration rate were more effective than the combination of uremic chloride granules and amlodipine. |
