In the course of the development of chronic obstructive pulmonary disease?AECOPD Acute chronic obstructive pulmonary disease?,symptoms such as fever,cough,sputum,and asthma can be significantly aggravated in the short term.The hospitalization is mainly Western medicine,but The prognosis is poor and the risk of death is high.Therefore,exploring new strategies and methods for the prevention and treatment of acute exacerbations of COPD is an urgent task for medical research in China.Purpose:The clinical efficacy and safety of Xuanfei Huatan Decoction were evaluated by comparing the changes of indicators before and after treatment in randomized controlled clinical trials.With a view to providing clinical reference for the treatment of AECOPD by Chinese medicine.Methods:?1?According to literature metrology and data mining research,improving lung function in patients with AECOPD is still a research hotspot.Airway inflammation-related factors IL-8,TNF-?,IL-6,IL-1?,etc.are the new hotspots and Xuanfei Huatan Decoction is mostly used clinically for patients with chronic cough,phlegm,asthma and other symptoms?phlegm turbidity obstructing lung syndrome?.This provides a reliable pre-research basis for the clinical research of its treatment of AECOPD?phlegm obstruction lung syndrome?.?2?In a randomized controlled clinical trial,120 AECOPD patients who met the inclusion criteria of this study were divided into a test group and a control group by random number table.The test group was given Xuanfei Huatan Decoction combined with conventional Western medicine treatment,and the control group was given Routine Western medicine treatment.After14 days of administration,Xuanfei Huatan Decoction was observed to improve the clinical symptoms,TCM syndrome scores,serum inflammation markers,blood gas analysis,and airway inflammation factors IL-1?,TNF-?,white of peripheral serum in patients with chronic obstructive pulmonary disease The effect of triene B4 and IL-8.?3?Further use network pharmacology methods to further analyze the potential molecular biological mechanism of Xuanfei Huatan Decoction in the treatment of COPD,and provide a theoretical basis for future advanced basic research and large center and multidisciplinary clinical research.Results:?1?Through bibliometrics and data mining studies,it has been found that improving lung function in patients with AECOPD is still a hot topic of research.Airway inflammation-related factors such as IL-8,TNF-?,IL-6,and IL-1?are currently clinically studied.The new hot spot and Xuanfei Huatan Decoction are derived from the clinical treatment of patients with chronic cough,sputum and asthma?phlegm turbid obstructive lung syndrome?.This provides a reliable pre-research basis for conducting clinical research on the treatment of AECOPD?phlegm,turbidity and obstructive lung syndrome?.?2?Through randomized controlled trials,it was found that after treatment,the total effective rate of 94.82%in the test group was significantly better than the total effective rate of 75.86%in the control group;the symptoms of the two groups after treatment,cough,sputum,wheezing,shortness of breath,spontaneous sweating,Appetite,bloating,and bowel movements were significantly improved compared with the treatment?P<0.05?;and the test group was superior to the control group in 7 dimensions of cough,sputum,wheezing,spontaneous sweating,appetite,bloating,and loose stools?P<0.05?.Blood gas analysis after treatment:Pa CO2,Pa O2,Sa O2were significantly improved in both groups than before treatment?P<0.05?;compared between the two groups,the test group was more obvious in the three aspects of Pa CO2,Pa O2,and Sa O2than the control group.?P<0.05?.Pulmonary function after treatment:FEV1%,FEV1/FVC,and VC%were significantly improved in the two groups than before treatment?P<0.05?;compared between the two groups,the experimental group was in both FEV1%and VC%.Compared with the control group,the increase was more obvious?P<0.05?.Blood routine results after treatment:WBC and NE%decreased significantly and LY%increased significantly in both groups?0.05?.Compared between the two groups,the test group was significantly more effective in terms of WBC and NE%than the control group,and it was also NE%?P<0.05?;The LY%test group was significantly higher than the control group?P<0.05?.Serum inflammatory factors IL-1?,TNF-?,leukotriene B4,and IL-8 after treatment:Serum inflammatory factors IL-1?,TNF-?,leukotriene B4,and IL-8 decreased significantly in both groups compared to before treatment,?P<0.05?.Comparing between groups,the decrease of IL-1?,TNF-?,leukotriene B4 and IL-8 in the test group was more significant than that in the control group?P<0.05?.?3?Research on Xuanfei Huatan Decoction through network pharmacology method found that it contains 62 effective chemical components.It was found that Xuanfei Huatan Decoction has 47 potential targets for COPD,with 12 pathways with significant?P<0.01?and false discovery rate?FDR<0.05?.Conclusions:Xuanfei Huatan Decoction can improve the clinical symptoms of patients with AECOPD?phlegm obstruction lung syndrome?by inhibiting the secretion of serum inflammatory factors IL-1?,TNF-?,leukotriene B4,and IL-8,with definite curative effect.At the same time,it is predicted that Xuanfei Huatan Decoction may regulate the expression of genes IL-6,IL-1?,TNF,etc.through multiple pathways including TNF signaling pathway and HIF-1 signaling pathway,affecting cell proliferation,inflammation,cell movement,etc.Process to achieve the purpose of treating the above-mentioned related diseases.However,due to shortcomings such as small sample size,lack of basic experimental verification,and lack of multi-disciplinary collaboration and cooperation,further in-depth research will be conducted in the future.
