Effect Of Shugan Jianxin Decoction On Health Status Of Patients With Chronic Heart Failure Complicated With Depression

Objective: By observing the clinical efficacy of Shugan Jianxin Decoction in the treatment of patients with chronic heart failure(CHF)complicated with depression and its impact on the health status of patients,to explore the effectiveness of its treatment,so as to provide theoretical basis and clinical guidance for the clinical treatment of heart failure complicated with depression with traditional Chinese medicine.Methods: After the test of inclusion and exclusion criteria,60 patients were randomly divided into two groups,30 cases in the experimental group and30 cases in the control group for 8 weeks of clinical intervention.The control group was treated with routine anti heart failure treatment of Western medicine;The experimental group was treated with routine anti heart failure treatment of Western medicine + Shugan Jianxin decoction.The changes of cardiac function grade(NYHA),N-terminal pro-B type natriuretic peptide(NT-pro BNP)level,left ventricular ejection fraction(LVEF),end diastolic left ventricular diameter(LV)and left atrial diameter(LA),six minute walk test(6MWT)distance,Hamilton Depression Scale(HAMD),Euro Qol Five Dimensions Questionnaire(EQ-5D)score and TCM syndrome score were observed before and after treatment,The differences of index levels and scale scores between the two groups before and after treatment were analyzed to evaluate the effect of Shugan Jianxin Decoction on patients with heart failure complicated with depression and its impact on health status.Results: The results showed that the cardiac function grade(NYHA),BNP level,6MWT distance,HAMD and EQ-5D scores of the two groups were improved after treatment.The total effective rate of improving cardiac function in the experimental group was 93.3%,which was significantly higher than 80.0%in the control group(P > 0.05).The total effective rate of TCM syndrome score improvement in the experimental group was 96.7%,which was significantly higher than 73.3% in the control group(P < 0.05).After treatment,compared with before treatment,the LV level in the control group was not significantly improved(P > 0.05),but in the experimental group was significantly improved(P < 0.05),the levels of LA and EF in the two groups after treatment were significantly improved compared with those before treatment(P < 0.05).There was no significant difference between the experimental group and the control group after treatment(P > 0.05),but the overall level was better than that in the control group.Compared with the control group,the BNP level and 6MWT distance in the experimental group were significantly improved after treatment(P < 0.05).After treatment,HAMD and EQ-5D scores in the experimental group were significantly improved compared with those before treatment(P < 0.05),and the curative effect was better than that in the control group(P < 0.05).Conclusion: Both groups can improve the cardiac function and exercise tolerance of patients with heart failure combined with depression to a certain extent,and improve the depressive state,but the curative effect of the experimental group is more significant,especially in terms of clinical symptoms of traditional Chinese medicine,6MWT distance and health state.Therefore,Shugan Jianxin Decoction is effective in the treatment of patients with chronic heart failure complicated with depression,which belongs to deficiency of heart and kidney yang and stagnation of liver qi.It is significantly better than the conventional anti heart failure treatment of Western medicine,and does not increase adverse reactions.It can be popularized in clinical application.
