| ObjectiveThe distribution law of clinical investigation in patients with heart failure of TCM syndrome elements, grasp the core of syndrome elements in patients with heart failure,and provide theoretical basis for the remarkable curative effect formula of Jian Xin Decoction.And through the observation of Jianxin Decoction effect on quality of life in patients with heart failure, further confirmed the efficacy of Jianxin Decoction to improve the quality of life.MethodThis thesis consists of two parts: 1 clinical investigation: through the investigation of the syndrome in patients with heart failure syndrome factors in 87 patients, found the existence of rules; Experimental Study 2: selected patients with heart failure, cardiac function grade II 70 cases, were randomly divided into control group and treatment group, the control group treated with western medicine routine to correct heart failure treatment, the treatment group in the control group based on the use of Chinese medicine compound Jianxin Decoction, one dose daily, divided two times clothes,observation of a course of treatment(four weeks). Comparison of treatment of cardiac function before and after classification, LVEF value, BNP, 6minutes walking distanceand quality of life scores. The obtained data were analyzed statistically by SPSS17.0 software.Result1. In the surveyed 87 cases of heart failure patients, disease is the most common syndrome factors of Qi deficiency and blood stasis, accounted for more than90.8%,83.9%, followed by drinking water, accounting for more than 55.2%, again for the turbid phlegm, yin deficiency, Yang deficiency. Qi deficiency and blood stasis syndrome factors of disease can be found in the different classification of cardiacfunction in patients, drinking water, turbid phlegm, yin deficiency disease of syndrome factors are scattered in different classification of cardiac function in patients with Yang deficiency syndrome factors, more common in patients with cardiac function of grade IV in. The disease location factors of syndrome is the most common heart, accounted for 96.6%,followed by the spleen, lung, kidney, liver. The heart function of patients with II stage,disease location as the heart, lung, spleen involved, the heart function of patients with III stage, disease location as the heart, lung, spleen, kidney involved, the heart function of patients with IV stage, disease location for heart, kidney,lung, spleen to liver, scattered in patients cardiac function classification.2. The control effect of heart function group and treatment group comparison of P>0.05,no statistical significance. The comparison between the groups before and after treatment: the value of LVEF is improved, but without statistical significance(P>0.05);the level of BNP decreased significantly(P<0.01); 6 minutes walking distance increased(P<0.01); improve the quality of life(P<0.05). Treatment group before and after treatment: the value of LVEF is improved, but without statistical significance(P>0.05);the level of BNP decreased significantly(P<0.01); 6 minutes walking distance increased(P<0.01); improve the quality of life(P<0.05). Comparison:treatment group and control after treatment LVEF value showed no statistical significance(P>0.05); BNP decreased significantly(P<0.01); 6 minutes walking distance increased significantly(P<0.01); quality of life improved significantly(P<0.05).Conclusion1. In 87 cases of patients with chronic heart failure, syndrome of Qi and blood stasis is the main. Mild to moderate heart failure patients, the main disease is the heart; in severe heart failure patients, the main disease is in heart and kidney.2. Jianxin Decoction combined with routine treatment for heart failure to correct within a short period of time(around) value was not significantly improved on congestive heart failure and cardiac function in patients with LVEF, but it couldeffectively decrease the level of BNP, increase 6 minutes walking distance, improve the quality of life. |
