| Objective:Through clinical research to observe the curative effect of Jianxin Decoction treatment of qi deficiency and blood stasis in patients with chronic heart failure,the simultaneous detection of the plasma endothelin1(ET-1)and calcitonin gene-related peptide(CGRP),and explore the possible mechanism and target of the treatment of chronic heart failure with Qi deficiency and blood stasis.Method:Randomly divided into standard average 40 patients into two groups,the experimental group and control group(n=20),the Chinese heart failure guidelines(2014)in the standardization of the recommended treatment as the basis of 40 patients with heart failure therapy,treatment group on this basis to take the Jianxin Decoction,two groups are in 28 days for observation period,the statistics of two groups of patients before and after the treatment related indicators,including cardiac function,TCM syndrome integral,Lee's heart failure points,the quality of life score of Minnesota(MLHFQ),left ventricular ejection fraction(LVEF),left ventricular diastolic diameter(LVED)end,serum brain natriuretic peptide(BNP)and endothelin-1(ET-1),calcitonin gene related peptide(CGRP).Result:1.On two groups of patients before treatment carried out a statistical analysis on the basic situation,including gender,age,course of the disease,cardiac function classification,and the primary disease,etc.,through the statistical analysis has no obvious difference(P>0.05),between the two way comparable.2.The clinical efficacy of:(1)cardiac function curative effect comparison: there are differences between the two groups of cardiac function after treatment curative effect(P<0.05),the team is better than that of control group,two groups total effective rate after treatment no difference(P>0.05),the significant efficiency are similar between the two groups after treatment(P<0.05),the team efficiency is 60%,the control group was 20%,the team is better than the control group;(2)the TCM syndrome integral,Lee's score and quality of life score comparison: heart failure treatment within two way to compare scores were significantly lower than before treatment(P<0.01),the treatment of patients with a comparison between the two way show the team after scoring points and Lee's heart failure syndromes are lower than the control group(P<0.05),while the quality of life score decreased more significantly(P<0.01);(3)LVED,LVEF value change: the two groups after treatment LVED,LVEF value than before treatment were obviously improved,each comparative difference was statistically significant(P<0.01),the two way improve after treatment group were better than control group(P <0.05).3.The physical and chemical indicators:(1)After treatment,the serum BNP level was significantly decreased after treatment(P<0.01),and the serum BNP was lower than the control group(P<0.05).(2)After treatment,the level of ET-1 in the study group was significantly lower than that before treatment(P<0.01),and the level of ET-1 in the control group decreased after treatment(P<0.05),and the serum ET-1 in the group after treatment was significantly lower than that in the control group(P<0.01).(3)After treatment,the CGRP level of the group was significantly higher than that before treatment(P<0.01),and the CGRP level in the control group was higher than before treatment(P<0.05).The serum CGRP was significantly higher than the control group(P<0.01).4.In the process of this experimental study,two groups of patients were not significant due to medication side effects and no tolerance,etc.,monitoring of 40 patients with blood,urine,stool routine,and liver and kidney function were not seen significant change and abnormal obviously.Clinical use of Jianxin Decoction is safe.Conclusions:This test by observing the Jianxin Decoction to qi deficiency and blood stasis type of clinical curative effect of patients with chronic heart failure,found on the basis of the standardized treatment of western medicine combined with healthy heart soup,can more effectively improve cardiac function in patients with heart failure,reduce the score of TCM syndrome integral,Lee's heart failure,make the quality of life score significantly reduced,increased LVEF and LVEDd,reduce the serum BNP level,improve the quality of life of patients.Studies have shown that healthy heart soup can significantly reduce the heart failure patients,the serum levels of ET-1 significantly improve the level of CGRP may prompt healthy heart soup treatment for heart failure mechanisms and targets is to improve endothelial function,inhibit circulatory endothelial cells release and elevated blood levels of CGRP and ET-1 and has good safety in clinical application. |
