Construction Of The First Draft Of Maternal Perinatal Group Health Care Service Standards

Objective:In view of the current situation of perinatal service demand for pregnant women,a preliminary draft of group health service model standard for perinatal women based on behavioral change theory was constructed to provide an effective theoretical basis for clinical medical staff to improve the quality of perinatal health service for pregnant women.Methods:1.Literature review method: to understand the research status of the model of perinatal health care at home and abroad and the method of constructing the first draft of the standard,and to determine the theoretical basis and items of the first draft of the standard of perinatal group health care service model for pregnant women based on the results of theoretical research;2.Questionnaire survey method: Taking the "Perinatal Health Service Demand Scale" as the research tool,the parturient women in different cities in Liaoning Province were selected to conduct a questionnaire survey,to collect the perinatal women's health service demands in a comprehensive and detailed way in Liaoning Province and add them to the items in the first draft of the standard;3.Expert group meeting method: on the basis of literature research and survey results,11 experts are invited to hold an expert group meeting,and the first draft of the standard is constructed according to the expert opinions;4.Delphi method:The Delphi method was used to conduct two rounds of expert consultation with 16 experts,and the first and final draft of the standard of the perinatal group health care service model for pregnant women in Liaoning Province was finally determined,and the weight of the indicators was calculated by data statistical method.5.Statistical methods: SPSS25.0 was used to establish a database,and descriptive statistical methods were used to describe the current situation of perinatal women's health service demand with X±S on the data collected from the questionnaire survey.For two rounds of Delphi method to entry,sorting and analyzing the data,calculate experts familiar with degree coefficient,coefficient of judgment,degree of authority coefficient,judge the reliability of expert advice and representation,the importance of calculating index average,standard deviation,coefficient of variation and so on,to build maternal group perinatal health care service pattern standard draft final version.Results:1.To establish behavioral change theory and group model as the basic framework of the standard model of maternal perinatal health service.2.Clarifying the current situation of the health service demands of pregnant and puerpera in the three stages of prenatal,peripartum and postpartum for the four service dimensions of physiology,psychology,environment and hospital.3.Confirmation of the first draft of the standards for health services,including 5 first-level indicators,20 second-level indicators,13 third-level indicators and 56 fourth-level indicators.4.The first draft was deleted through two rounds of Delphi method expert consultation,and the final draft of the standard of group health service mode for maternal and perinatal period was finally formed,including 5 first-level indicators,17 second-level indicators,29third-level indicators and 48 fourth-level indicators,and the average range of importance scores for all levels of indicators.At this stage,there are 16 original correspondence experts,aged 34-54 and working experience 11-28 years.The positive coefficient of the two rounds of expert letter consultation was 100% and 100%,the expert authority coefficient of the two rounds was 0.810,and the coordination coefficient of indicators at all levels after the two rounds of letter consultation was0.091 and 0.165,and the test results were statistically significant(P < 0.01),indicating that the consulting experts had good reliability and representativeness.Conclusion:By domestic and foreign literature analysis,questionnaire investigation,this study panel meeting method and Delphi method jointly build the first draft standard for maternal group perinatal health care service model has certain systemic and normative,standard draft for perinatal care services,provides the effective basis for standardized management,to improve the quality of the maternal perinatal health care service is of great significance.
