Assessment Of Health Management Effect On Hypertensive Patients Based On Care Home Mode

Objective:To investigate the effect of a care home health management mode on hypertensive patients,and to explore a new method suitable for the health management of hypertension patients in the community.Methods:A simple random sampling was used to select 200 hypertensive patients from Xuhui District Changqiao Community Health Service Center,Shanghai.In this study,100 patients were included in the experimental group and 100 patients in the control group,the experimental group adopted a home health management mode to manage patients,and the control group used the existing high blood pressure management methods.All patients were followed up for one year to observe the intervention effect,and all patients' BMI,blood pressure,blood lipids,glycated hemoglobin was detected,and a questionnaire survey on ability of daily life and life satisfaction were investiated in patients of two groups.Results:A total of 200 participants was included in this study.The mean age at baseline was 70.36 years,and there were 78 males(39%)and 122 females(61%).In the experimental group and the control group after intervention for hypertension knowledge population has promoted,the difference was statistically significant(P<0.05).After the intervention,the experimental group(except for smoking)adopt healthy behavior was improved obviously,the difference was statistically significant,P<0.05.In the intervention group patients,BMI decreased by 1.42 kg/m2,systolic blood pressure and diastolic blood pressure decreased by 23.52 mmHg and 11.23mmHg,high density lipoprotein cholesterol(HDL-C)increased 1.51 mmol/L,low density lipoprotein cholesterol(LDL-C)decreased 0.52 mmol/L,total cholesterol(TC)decreased by 0.57 mmol/L and triglycerides(TG)decreased by 0.96 mmol/L,and glycosylated hemoglobin(HbA1C)decreased 1.46 mmol/L.Daily living ability scores increased by 54.2,Lawton's daily living ability score decreased by 28.12,satisfaction improved by 60%.The total score on the self rating scale decreased by 25.72,the total symptom index(GSI)decreased by 0.18,the proportion of depressed patients with the average score of 2 points decreased by 20%,and the proportion of anxiety symptoms above 2 points(%)decreased by 11.3%after 1 year's management.In the control group,systolic blood pressure decreased by 9.23mmHg,diastolic blood pressure decreased by 21.2 mmHg,BMI decreased by 1.25kg/m2,high density lipoprotein cholesterol(HDL-C)increased 0.18 mmol/L.low density lipoprotein cholesterol(LDL-C)decreased 0.41 mmol/L,total cholesterol(TC)decreased three 0.55mmol/L,triglyceride(TG),glycosylated hemoglobin decreased 0.89mmol/L(HbA1C)decreased by 1.02mmol/L.Daily living ability scores increased by 27.3,Lawton's daily living ability score decreased by 24.5,satisfaction improved by 20.3%.The total score on the self rating scale decreased by 10.73.the total symptom index(GSI)decreased by 0.17,the proportion of depressed patients with the average score of 2 points decreased by 12.5%,and the proportion of anxiety symptoms>2 points decreased by 5%after 1 year's management.In the experimental group,systolic blood pressure,diastolic blood pressure,BMI,high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),total cholesterol(TC),triglycerides(TG),glycosylated hemoglobin(HbA1C)and Lawton's ADL score,ADL score,satisfaction,self rating scale total score,total symptom index score,proportion of depression score>2 GSI,and proportion of anxiety symptom scores>2 points were significantly improved,the difference between post-management and pre-management was statistically significant(P<0.05).In the control group,systolic blood pressure,diastolic blood pressure,BMI,high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),total cholesterol(TC),three glycerol(TG),glycosylated hemoglobin(HbAlC)and Lawton's ADL score,ADL score,self rating scale the total score,total symptom index score GSI indexes were significantly improved,the difference between post-management and pre-management was statistically significant(P<0.05).There was no significant difference in satisfaction score,the proportion of depression symptoms scores>2 and anxiety symptoms>2 points between post-management and pre-management.After intervention,systolic blood pressure,diastolic blood pressure,high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),total cholesterol(TC),three glycerol(TG),glycosylated hemoglobin(HbA1C),ADL score,Lawton's score and ability of daily life satisfaction,self rating scale total score index improved significantly in the experimental group than that in the control group(P<0.05).However,the difference of the total symptom index,proportion of depression symptoms socres>2 and anxiety symptoms>2 points was not statistically significant(P>0.05)between the experimental group and the control group.
