Clinical Observation Of Combined Acupuncture And Medicine In The Treatment Of Premature Ovarian Insufficiency Of Liver-kidney Yin Deficiency Syndrome

Purpose:To compare the clinical efficacy of acupuncture combined with traditional Chinese medicine（combined internal and external treatment）and oral Chinese medicine alone（internal treatment）in the treatment of Premature Ovarian Insufficiency with liver and kidney yin deficiency,and to observe the changes in the TCM syndrome score（including major and minor syndrome;TCM syndrome individual score and total score）,survival quality scale score,serum sex hormone level[including follicle stimulating hormone（FSH）,luteinizing hormone（LH）,estradiol（E₂）,and anti-mullerian hormone（AMH）]before and after treatment.This study was conducted to verify the efficacy of the combination of acupuncture and medicine in the treatment of Premature Ovarian Insufficiency with liver and kidney yin deficiency compared with traditional Chinese medicine,and to provide a new idea and insight for the future clinical treatment of Premature Ovarian Insufficiency.Material and method:The study population consisted of 60 patients with Premature Ovarian Insufficiency with liver and kidney yin deficiency evidence who attended the gynecology clinic of the Affiliated Hospital of Liaoning University of Chinese Medicine and met the inclusion criteria,with the case collection starting and ending from October 1,2018 to October 1,2020.The patients were randomly divided into 30 cases in the experimental group and 30 cases in the control group by simple randomization method in the order of time of visit.The 2 groups of study subjects were treated with combined internal and external treatment and internal treatment alone.3 months was considered as 1 course of treatment,and the patients were observed before and after treatment for TCM evidence scores,patient quality of survival scale scores,and changes in serum hormone levels.Results:1.Comparison of curative effect of TCM:In the control group,the total effective rate was86.67%and the ineffective rate was 13.33%,and the effective rate was 96.67%and the ineffective rate was 3.33%.It indicated that the curative effect of internal and external treatment was better than that of internal treatment with oral Chinese medicine alone（P<0.01）.2.Comparison of the curative effect of serum hormone FSH:In the control group,the total effective rate was 73.33%,the ineffective rate was 26.67%,the effective rate was 93.33%,and the ineffective rate was 6.67%.It indicated that the therapeutic effect of internal and external treatment was better than that of internal treatment with oral Chinese medicine alone in improving serum FSH levels（P<0.05）.3.The scores of quality of life scale in the two groups were increased after treatment（P<0.05）,indicating that the quality of life in the two groups was improved after treatment,and the effect was better in the test group.4.After 1 course of treatment,the serum FSH and LH values of patients in both groups decreased（P<0.05）,which was statistically significant;indicating that the serum FSH and LH of patients in both groups improved,and the experimental group had better results.5.There was no significant improvement in serum AMH in both the test and control groups before and after treatment（P>0.05）,which was not statistically significant,indicating that there was no significant efficacy of the two treatment modalities on the improvement of serum AMH levels in a short period of time.6.There was no significant improvement in serum E₂ in both the test and control groups before and after treatment（P>0.05）,which was not statistically significant,indicating that there was no significant efficacy of the two treatment modalities on the improvement of serum E₂ levels in a short period of time.Conclusion:1.The efficacy of the experimental group with combined internal and external treatment was better than that of the control group treated with internal treatment alone in most aspects, indicating that the treatment modality of acupuncture plus oral tonics was more effective overall.2.The combined internal and external treatment method in the experimental group could significantly improve the clinical symptoms of patients with Premature Ovarian Insufficiency due to liver and kidney yin deficiency,fully demonstrating the advantages of combined internal and external treatment in TCM.3.Both the internal treatment method and the combined internal and external treatment method could improve the survival quality scores of patients,and the combined internal and external treatment method was more effective.4.By observing the results of serum hormone levels（FSH、LH）,it can be objectively seen that the combined internal and external treatment method of acupuncture and medicine is more effective than the internal treatment method of traditional Chinese medicine alone in treating Premature Ovarian Insufficiency with liver and kidney yin deficiency evidence.
