Based On The Literature Research On The Medication Rule Of Diabetic Nephropathy And Edema

Purpose: Traditional Chinese medicine has been used to treat diabetic nephropathy and edema since ancient times.There are a hundred schools of clinical medicines contending,and empirical prescriptions and self-made prescriptions are endless,but there are few systematic summaries.The purpose of this study is to use statistical methods to summarize the collected literatures on the treatment of diabetic nephropathy and edema in traditional Chinese medicine in the past 15 years,discuss the efficacy of relevant prescriptions,and summarize the rules of medication for diabetic nephropathy and edema,in order to provide a basis for clinical diagnosis and treatment.Materials and methods: Computer retrieval of literature on the treatment of diabetic nephropathy and edema from January 1,2005 to January 1,2020,in China National Knowledge Infrastructure(CNKI),Wanfang Data Knowledge Service Platform(Wanfang Data),Weipu Network(VIP),China Biology Medicine disc(CBMdisc),import it into Note Express3.3 software to establish a database.Filter out duplicate literature titles,mark the literature that meets the inclusion criteria but not the exclusion criteria,and import the final included literature into Excel for further summary and sorting.And use SPSS26.0 to cluster the traditional Chinese medicines with higher frequency of use,discuss the statistical results,and draw conclusions.Result: After screening,there are 109 documents that meet the criteria and involve 32 types of syndromes.The syndrome type that appears most frequently is syndrome of yang deficiency of the spleen and kidney.Involving 119 traditional Chinese medicine compound prescriptions,64 of which clearly mention the name of the prescription,and the most frequently used prescription is Zhenwu Decoction.Contains 160 kinds of traditional Chinese medicine,with a total frequency of 1282 times,of which Astragalus is used 85 times,ranking first.Traditional Chinese medicine for the treatment of diabetic nephropathy and edema,the main properties are cold,warm,and flat,which account for 91.25%;sweet medicine and bitter medicine account for a larger proportion,followed by pungent medicine;21% of them are in the meridian Medicine can enter the liver meridian,and there are relatively more Chinese medicines that can enter the kidney meridian,lung meridian,and spleen meridian.Dividing the collected Chinese medicines into categories,the most used are tonic medicines,with a total of 413 occurrences,with a frequency of 32.22%,followed by diuretic and dampening medicines,with a frequency of 217 times.Traditional Chinese medicines used more than 10 times include 33 flavors such as Astragalus and Poria,which are commonly used drugs for the treatment of diabetic nephropathy and edema.The above-mentioned traditional Chinese medicine was analyzed by systematic clustering,and 10 medicine pairs with higher reliability were obtained,and 4 clustering prescriptions were obtained.Conclusion: 1.In clinical treatment of diabetic nephropathy and edema,the syndrome type most involved is syndrome of yang deficiency of spleen and kidney,followed by syndrome of deficiency of both qi and yin and blood stasis.2.The most frequent prescription for clinical treatment of diabetic nephropathy and edema is Zhenwu Decoction,followed by Wuling Powder,Wuling Powder and Wupi Yin and Huangqi Jianzhong Decoction.3.Astragalus,Poria,Alisma,Chinese Yam,Danshen,Atractylodes macrocephala and other 33 traditional Chinese medicines are commonly used drugs for clinical treatment of diabetic nephropathy and edema,and Astragalus is used the most frequently.4.Traditional Chinese medicine for clinical treatment of diabetic nephropathy and edema.Its properties are mainly cold,warm,and flat;the taste of the medicine is mainly sweet,bitter,and pungent;the main meridian is the liver meridian,kidney meridian,lung meridian,and spleen meridian.5.Traditional Chinese medicines for clinical treatment of diabetic nephropathy and edema are mainly in the categories of tonic drugs,diuretic and dampening drugs,blood circulation and blood stasis drugs,and the qi tonic drugs are used the most frequently among the tonic drugs.6.Among the top 5 drug pairs with higher reliability obtained by hierarchical clustering analysis,Rehmannia glutinosa and Moutan cortex are suitable for diabetic nephropathy with edema and kidney yin deficiency.Cornus and Rehmannia glutinosa are suitable for the deficiency of liver and kidney yin in this disease.Chinese yam and dogwood can be used for diabetic nephropathy with edema and obvious proteinuria.Poria and Alisma can be used for diabetic nephropathy in all stages of edema.Angelica and Chuanxiong are suitable for the disease when the water and blood stasis are mutually condensed.Among the 4 cluster prescriptions obtained,the first cluster prescription is a modified formula of Buyang Huanwu Decoction,which can treat diabetic nephropathy and edema caused by early Qi and Yin deficiency,kidney Yin deficiency,and water stasis.The second cluster prescription is Jishengshenqi Pill,which can treat diabetic nephropathy and edema caused by spleen and kidney yang deficiency,kidney yang deficiency,and water stasis.The third cluster are genuine Wulingsan Wu Decoction of the addition and subtraction can Spleen deficiency,diabetic nephropathy deficiency caused by edema;The fourth cluster prescription is composed of Taizi,licorice,earthworm,and ghost arrow feathers,which can cure diabetic nephropathy and edema caused by qi and yin deficiency,blood stasis,and damp heat.
