| Objective:To explore the intervention effects of rational emotional behavior therapy on anxiety and depression in patients with early threatened abortion after in vitro fertilization-embryo transfer(IVF-ET),as well as the effects on sleep quality, quality of life, and early miscarriage rate.Through the evaluation of patients’ satisfaction with rational emotional behavior therapy,we aim to provide a theoretical basis for constructing a psychological intervention program for patients with early threatened abortion after IVF-ET.Methods:A quasi-experimental study was used. The convenience sampling method was used to select 124 patients with early threatened abortion after IVF-ET as the research subjects, who were treated in a reproductive center in Lanzhou from December 2020 to September 2021.All the subjects met the inclusion and exclusion criteria. The patients were grouped by visiting time: patients who admitted to the hospital from December 2020 to April 2021 were included in the control group(n=62);the patients admitted to the hospital from May 2021 to September 2021 were included in the experimental group(n=62).The control group was treated with routine nursing, while the experimental group was treated with rational emotional behavior therapy in addition to routine nursing. The rational emotional behavior therapy was performed30~60 min each time, twice a week, for a total of 4 weeks. The Self-rating Anxiety Scale(SAS),Self-rating Depression Scale(SDS),Pittsburgh Sleep Quality Index(PSQI),and Fertility Quality of Life questionnaire(Ferti Qo L)were used to measure the scores of anxiety, depression, sleep quality, and quality of life before the intervention(on the day of admission)and after the intervention(4 weekends after the intervention). And the satisfaction questionnaire on the rational emotional behavior therapy in the experimental group was collected at the fourth weekend of intervention. At the twelfth week of gestation, the early miscarriage rate in the two groups were collected. In this study,SPSS 25.0 was used for statistical analysis. Results:After the intervention,a total of 115 patients with early threatened abortion after IVF-ET completed data collection, including 58 in the experimental group and 57 in the control group. The specific research results were as follows:(1)Before the intervention, the age,BMI,ethnicity, place of residence, education level, occupation, family average monthly income, marital history, and other baseline data were not significantly different(P>0.05).The baseline data of the two groups were comparable.(2)Comparison of the two groups before the intervention: there was no significant difference in the SAS,SDS,PSQI and Ferti Qo L scores of the experimental group and the control group(P>0.05),and the groups were comparable.(3)Comparison of anxiety scores, comparison between groups: the SAS score of the experimental group was significantly lower than that of the control group after the intervention(P<0.05).Intra-group comparison: the SAS score of the experimental group was significantly lower after the intervention(P < 0.05).There was no significant difference in the SAS scores of the control group before and after the intervention(P>0.05).(4)Comparison of depression scores, comparison between groups: the SDS score of the experimental group was significantly lower than that of the control group after the intervention(P<0.05).Intra-group comparison: the SDS score of the experimental group was significantly lower after the intervention(P < 0.05).There was no significant difference in the SDS score of the control group before and after the intervention(P>0.05).(5)Comparison of sleep quality scores, comparison between groups: the PSQI score of the experimental group was significantly lower than that of the control group after the intervention(P < 0.05).Intra-group comparison: the PSQI score of the experimental group was significantly lower after the intervention(P<0.05).There was no significant difference in the PSQI score of the control group before and after the intervention(P>0.05).(6)Comparison of quality of life scores, comparison between groups: the Ferti Qo L score of the experimental group was significantly higher than that of the control group after the intervention(P<0.05).Intra-group comparison: the Ferti Qo L score of the experimental group was significantly higher after the intervention(P<0.05).There was no significant difference in the Ferti Qo L scores of the control group before and after the intervention(P>0.05).(7)Early miscarriage rate: The early miscarriage rate in the experimental group(12.07%)was lower than that in the control group(21.05%),but the difference was not statistically significant(P>0.05).(8)The satisfaction of the patients in the experimental group to rational emotional behavior therapy was 98.3%~100%.Conclusion:Rational emotional behavior therapy can effectively relieve anxiety and depression in patients with early threatened abortion after IVF-ET,and improve their sleep quality and quality of life.Rational emotional behavior therapy failed to reduce the early miscarriage rate and its impact on the early miscarriage rate needs further study. The patients in the experimental group had higher satisfaction with rational emotional behavior therapy. This study provides a reference for the construction of psychological intervention programs for patients with early threatened abortion after IVF-ET. |
