| Objective:Based on the theory of" combination of disease-syndrome-symptoms ",the effect of the comprehensive TCM program on the TCM syndrome score,wound area,wound healing rate,ABI,quality of life scores,HbAlc,serum TNF-α,serum VEGF,and comprehensive curative effects on patients with DFU were observed.The clinical value of TCM comprehensive program for DFU was explored,and a theoretical basis for clinical TCM treatment of DFU was provided.Methods:A total of 120 patients from Chengdu University of TCM Hospital,Meishan TCM Hospital,and Suining TCM Hospital were selected and divided into a control group and an experimental group according to the principle of randomization.There are 60 patients in each group.After joining the group,the two groups received unified basic and Western medical treatment.A comprehensive DFU TCM program(oral herbal medicine+ear point pressing bean+acupuncture+acupoint injection)was added to the experimental group based on the control group,8 weeks in total.The changes in TCM syndrome score,ulcer wound area,wound healing rate,ABI,multidimensional quality of life score,Hb Alc,serum TNF-α,and serum VEGF before and after treatment were observed.Using SPSS 25.0 software for statistical analysised.Results:(1)Impact on TCM syndrome score:After treatment,the systemic and local symptoms of diabetic foot ulcers in the two groups were significantly lower than before,and there were significant statistical differences between the two groups(P<0.01).(2)Wound area,wound healing rate,and impact of ABI:Wound area between the two groups decreased significantly after treatment,but there was no statistical difference between the two groups(P>0.05).There was no statistical difference in wound healing rates at 4 weeks(P>0.05),but there was a statistically significant difference in wound healing rates between the two groups 8 weeks after treatment(P<0.05).The ABI of the two groups improved,but there was no statistical difference between the two groups(P>0.05).(3)Impact of quality of life on multidimensional scores:After treatment,the total quality of life scores of the two groups decreased significantly,and comparisons between the groups were also statistically significant(P<0.01).In a one-way dimension comparison,the score for each dimension after treatment was lower than before,the difference between the two groups in the physiological dimension was statistically significant(P<0.05),and significant between the two groups.There was statistical significance.There was no statistical difference between the two groups in terms of psychology and social aspects(P<0.01),but in terms of treatment(P>0.05).(4)Effect on blood-related laboratory indicators:After treatment,two groups of HbAlc were compared and the results showed that the difference was statistically significant(P<0.05);TNF-α decreased and VEGF elevated after 4th and 8th week of treatment(P<0.01).There are significant statistical differences between groups(P<0.01).(5)Effect on overall therapeutic effect:After 8 weeks of treatment,the overall efficacy rate of the control group was 78.6%,and the overall efficacy rate of the experimental group was 94.4%.The difference between the two groups was statistically significant(P<0.05).Conclusions:A comprehensive TCM program based on the theory of "combination of disease-syndrome-symptoms" effectively improves clinical symptoms in DFU patients,shortens wound healing time,improves patient quality of life and reduces glycated hemoglobin,reduces the body’s inflammatory response,promotes the proliferation of wound vascular endothelial cells,and improves comprehensive clinical efficacy.No significant side effect records were recorded during the experiment,indicating that the program is highly safe. |
