Objective:(1)To evaluate the clinical efficacy and safety of modified Liangxuexiaofeng powder in the treatment of recurrent psoriasis vulgaris(blood heat syndrome).(2)By comparing the expression levels of t-bet and GATA-3 in healthy people and patients with recurrent psoriasis vulgaris(blood heat syndrome),the immunological characteristics of recurrent psoriasis vulgaris were investigated from the perspective of T-bet /GATA-3 balance.(3)By detecting the expression levels of T-bet and GATA-3 before and after the treatment of modified Liangxuexiaofeng Powder,the immunological mechanism of modified Liangxuexiaofeng Powder in the treatment of psoriasis vulgaris(blood heat syndrome)was clarified.Method: Observational study:A total of 72 patients with recurrent psoriasis vulgaris(blood heat syndrome)were randomly divided into experimental group(n =36)and control group(n = 36).The experimental group was treated with modified Liangxuexiaofeng powder,and the control group was treated with Avec A capsule.Both groups were treated for 8 weeks.PASI score,BSA score,DQOLSand adverse events were evaluated before and after treatment to determine the clinical efficacy and safety.Laboratory testing:Ten people were randomly selected from the experimental group and the control group,and 10 healthy people were included as the healthy group.The expression levels of t-bet and GATA-3 in the peripheral blood of the subjects were detected by real-time fluorescence PCR and Western Blot.Results: Analysis of clinical observation indexes: After 8 weeks of treatment,the PASI score and BSA of the experimental group and the control group were significantly decreased compared with before treatment(P<0.05),while the DQOLS score was significantly increased compared with before treatment(P<0.05).After 8weeks of treatment,there was no difference in the index values between the two groups(P>0.05).Two groups total effective contrast: the experimental group was 96.77%,the control group was 96.67%,no difference between groups(P>0.05).Comparison of the total adverse reaction rate between the two groups: the experimental group was 6.45%,and the control group was 30%,the experimental group was lower than the control group(P<0.05).Analysis of flow cytometry results:(1)The expression level of T-bet in peripheral blood of patients with recurrent psoriasis vulgaris(blood heat syndrome)was significantly higher than that of healthy people,and the expression level of GATA-3was significantly lower than that of healthy people(P<0.01).(2)The expression of Tbet and GATA-3 in peripheral blood were decreased and increased in both experimental and control groups after treatment(P<0.05).(3)There was no significant difference in Tbet values between the experimental group and the control group(P>0.05).Conclusion:Patients with recurrent psoriasis vulgaris have an imbalance of Tbet/GATA-3 in peripheral blood: high expression of T-bet and low expression of GATA-3,which may be related to the pathogenesis of psoriasis.The modified Liangxuexiaofeng powder is safe and effective in the treatment of blood heat syndrome of recurrent psoriasis vulgaris.The immunological mechanism may be to restore the balance of T-bet/GATA-3 by reducing the expression level of T-bet and increasing the expression level of GATA-3,so as to achieve the therapeutic purpose.
