| China has entered a new development stage of the 14 th five year plan.At this stage,"promoting the high-quality development of medical security" is an important content of our era construction.Both the "14th five year plan" and the "outline of long-term goals for2035",have emphasized that China should uphold the public welfare attribute of basic medical and health undertakings,take improving medical quality and efficiency as the guidance,Adhere to the dominant position of public medical institutions,supplemented by non-public medical institutions.We are plan to steadily expand the coverage of contracted services for family doctors in urban and rural areas,and improve the quality of contracted services.In 2011,Shanghai,as one of the first cities in China to implement family doctor contract services,insisted on the implementation of family doctor policy as an important starting point for deepening medical reform.In 2020,Shanghai further clarified and standardized the signing requirements,service methods,personnel responsibilities and assessment basis of family doctor signing services,o as to further improve the family doctor signing services in an orderly manner.Adding the family doctor system to the existing grass-roots medical and health service model in China is an important medical and health reform.The introduction of this system can promote the quality and service effect of residents’ health management and graded diagnosis and treatment.It can improve management and hierarchical diagnosis and treatment,and meet the growing medical and health needs of the population.However,there are still obstacles and difficulties in the signing service of family doctors in China.The future is very bright,but we can not ignore the problems existing in the promotion and implementation of China’s family doctor system at this stage,and further improve the signing rate of family doctors.Firstly,in this paper,we introduce the theoretical and practical background of the research problems,and then introduce the significance of this research under the background of the times and the research status at home and abroad,research methods and research ideas.Secondly,it defines the related concepts such as family doctor system,family doctor signing contract and residents’ willingness to sign contract,and combs the relevant theories such as individual difference theory,cognitive theory and expected value theory,so as to lay the theoretical foundation of this paper.Then,the text of the family doctor system is combed to further clarify the relevant policy requirements of family doctors and the service content of family doctors.Based on literature review and theoretical support,taking family doctors and residents in J District of Shanghai as survey and interview objects,and base on empirical research methods,we reveal the relationship between residents’ individual differences,cognition,expected value and family doctors’ willingness to sign contracts,and finds that residents’ individual differences,cognition and expected value have a positive impact on signing behavior.In addition,the study also found that the family doctor system has some problems,such as less personalized services,insufficient publicity and weak supporting mechanism.Finally,based on the results of literature analysis and experimental data analysis,this paper puts forward relevant suggestions so as to improve the signing rate of residents’ family doctor system,such as enriching service connotation,strengthening publicity and improving supporting mechanism. |
