Object:To explore the application and effect evaluation of the whole course management model in patientswith schizophrenia.Method:From January 2019 to February 2020,129 Han schizophrenic patients and their main caregive rs(family or guardians)who were hospitalized in Shihezi Oasis Hospital were randomized into a whole course management model group(study groups;n=64)and a conventional treatment group(stan dard care group;n=65).Both groups were followed up for 1 year,and follow-up for another year af ter discharge.Two groups of patients and their main caregivers used self-compiled general demogra phic data to fill in the basic information.Using Positive and Negative Symptom Scale(PANSS Sc ale),Social Function Deficiency Screening Scale(SDSS Scale),Activity of Daily Living Scale(A DL Scale)before the intervention,3 months,6 months,and 12 months after the intervention to ev aluate patients’psychotic symptoms and functions.Medication Compliance Scale(MMAS-8 scale)an d recurrence rate were assessed at 3,6,and 12 months after discharge.The main caregivers of th e two groups of patients used the Depreciation-Discrimination Scale(PDD Scale)to assess stigma at the above time points.5 cases were lost during the follow-up process because of relocation,ch ange of contact information,etc(2 cases in the study group and 3 cases in the control group)and124 cases were successfully completed(including 21 cases in hospital and 103 cases out of hospi tal).After discharge from the hospital,a total of 103 cases were followed up for 1 year(includin g 18 cases).The cases lost to follow-up have been eliminated in the following data analysis.SPS S20.0 statistical software was used for data input,sorting and statistical analysis.Statistical methods includedc~2test or nonparametric test,t test,and repeated measures analysis of variance.If the h ypothesis test of spherical symmetry is satisfied,two-factor ANOVA is carried out.If the hypothesis test ofspherical symmetry is not satisfied,Greenhouse-Geisser method is used to correct the degre e of freedom.Graph Pad Prism 9 was used to plot a statistical graph of repeated measurements,P<0.05 indicates that the difference is statistically significant.Result:1.There was no difference between the two groups in basic line(P>0.05).2.PANSS and negative symptom score of two groups,had a group time point interaction ef fect(P=0.018,0.0007).The psychopathological scores of the two groups were different after 6 and12 months intervention(P=0.0006,P<0.001).After 12 months intervention,the negative symptom score of the two groups has significant difference(P=0.002).3.SDSS and ADL scores of two groups had a interaction between group and time(P<0.001,P=0.008).SDSS scores of the study group were lower than control group after 6th and 12th mon ths of intervention and the difference was significant(P=0.006,P<0.001).ADL points has Signific ant difference between two groups after 12 months of intervention(P<0.001).4.After the intervention,the compliance of the study group was better than control group and the difference was significant(P<0.01).5.There was no significant difference in the recurrence rate between the two groups(P>0.05).6.The PDD score of two groups has group-time interaction effects(F=22.942,P<0.001).Aft er 6 and 12 months,the stigma of the study group was lower than control group,and the differe nce was significant(P<0.001).Conclusion:Compared with the conventional treatment mode,the whole course management modelcan not only further improve psychotic symptoms and functional impairment of patients with schizophrenia,improve the drug compliance of patients,but also reduce the stigma of main caregivers and prom ote the comprehensive recovery of patients.
