| ObjectiveTo observe the clinical efficacy and safety of fire needle therapy of spastic upper limb paralysis in the convalescent stage of ischemic stroke through a controlled clinical trial, and to provide an effective basis for the formulation and optimization of the treatment of ischemic stroke recovery spastic upper limb paralysis by comparing it with filiform needle therapy.MethodsAll patients in this study were from the Encephalopathy Acupuncture Center of the Second Affiliated Hospital of Tianjin University of Traditional Chinese Medicine.According to the random number table method, researcher puts 60 patients into treatment group and coutrol group, each group has 30 patients. The two groups were given different acupuncture methods on the basis of western medicine basic treatment.Choose acupoints are all from "Tiao Shentongluo" method and mentor clinical experience effect, acupoints as follows: the top line, oblique line, near the top line 1,near the top line 2,double side Feng Chi、affected side Jian Yu,Jian Liao,Tian Jin,Wai Guan,Qu Chi,Bi Nao,Ba Xie,as well as the partial reinforcement nodes, pressure points, finger section near the middle of back side of the interphalangeal joints. In the treatment group, the acupuncture on the head acupoints were performed with filiform needles, and the acupuncture was continued for 30 seconds after receiving qi.When the acupuncture was performed, the patients were instructed to move the corresponding parts as far as possible, and then retained the needles for 30 minutes. The other acupoints were punctured with fire needles, each acupoint was punctured once, and no needles were left. The operation methods of the control group were as follows: acupuncture on the head acupoints was the same as that of the treatment group. The rest of the acupoints were acupuncture with filiform needles, and acupuncture was left for 30 minutes after receiving qi.The control group was treated once a day from Monday to Saturday, one week as a course of treatment. The treatment group was treated once every other day from Monday to Saturday, one course of treatment was once a week, and both groups were treated for 3 courses.Patients in both groups were compared before and after treatment with FMA,MBI and MAS.Of the two treatment regimens were evaluated for clinical efficacy of ischemic stroke recovery of upper limb spastic paralysis based on improvement indicators.Results1.Two groups of patients’ age, gender, site of spasm and relative duration of disease don’t have statistically significant differences.(P>0.05).2.Before the treatment,it doesn’t have any statistically significant differences between the two groups of upper limb FMA,MBI and upper limb MAS.(P>0.05).3.After treatment, the FMA,MBI and MAS scores of the two groups’ patients were better than those before treatment, and the improvement was statistically significant(P<0.05),and the treatment group was better than the control group.4.After treatment, the effective rate of 93.33% in the treatment group is much higher than that of the control group,76.67%,and there is a statistical difference(P<0.05).Conclusion1.Both fire needle therapy and filiform needle therapy can improve the state of spasmodic paralysis of upper limb in patients with convalescent ischemic stroke, and improve the quality of life of patients.2.In terms of improving the upper limb motor function of patients, improving the ability of daily living, and reducing the spasticity level of patients, fire needle therapy is superior to filiform needle therapy.3.In terms of the total clinical efficacy, fire needle therapy is better than filiform needle therapy.4.Fire needle therapy has significant efficacy, high safety, strong stimulation, short time, easy operation, low requirements on patients’ body position, low cost, and is worth promoting in clinical practice. |
