Objective:Through the animal experiment and collection of clinical case, analysised the effect of fire needle therapy on post-stroke limb spasticity in patients、 the effect of fire needle therapy on the animal model’s brain excitability and/or inhibitory neurotransmitter, to explore the molecular mechanism.Methods:This research contained two aspects:clinical research and animal studies:1. clinical researchCollect70cases, tratified mild, moderate, severe in3layers, divided random into two groups,①fire needle group,②Baclofen group,30cases in each group.Interventions:③All patients received basic treatment:gave symptomatic&Supportive treatment, including the control of blood pressure, blood sugar, blood lipids, body temperature, oxygen, and maintain water, electrolyte and acid-base balance, gave taspirin use to improve blood circulation in the brain and the brain cells activated etc. All patients were doing the basic rehabilitation (including the placing of limbs in good position, bedside sitting balance, horizontal movement, balance training, standing bed training, active and passive movement, occupational therapy, etc.(2) fire needle groupSelected acupoint around the spasm the muscles:Jianyu(LI15)、qiuxu,(GB40)、zulinqi (GB41), etc. Acupuncture once every other day with15times as a course.(3) Baclofen groups,baclofen5mg P.0. TID, increased the dose depended on patient response gradually until the desired effect, the maximum dose of80mg/d. end of the trial after30days2. animal experimentsAfter1week adaptive feeding,90SPF SD rats were randomly grouped into4groups:①the normal group、②the model group、③fire needle group,④baclofen group.Modeling methods:on the basis of the improved Ngaaswaa’s method, used intravascular tying block method to build model.Interventions:①the normal group、②the model group caught empty, without any treatment.③fire needle group:Fire Needle acupoint around the spasm the muscles, acupuncture once every other day with15times as a course.④baclofen group:0.3mg/kg daily,28days.Results:(1) Fire needle can reduce the diagnostic efficacy score of patiant.(2) Fire needle can reduce Ashworth, CSI score, improve the FMA score, to ease spasticity.(3) Fire needle can improve the coordination ability and its improvement trend of the model rat.(4) Fire needle can extend H reflex latency、reduce the volatility of the H reflex in the model rat.(5) Fire needle can reduce glutamate (GluK increase the glycine (Gly) in hippocampus of the model rat.Conelus ion:(1) Fire needle can alleviate spasticity in patients with limb spasticity after stroke;(2)intravascular tying block method is a valid way to build the animal model;Fire needle can reduce glutamate (Glu)、increase the glycine (Gly) in hippocampus、improve the coordination ability and its improvement trend、extend H reflex latency、reduce the volatility of the H reflex of the model rat.
