ObjectiveUsing Jiawei Wutou Decoction to treat painful diabetic peripheral neuropathy of yang deficiency and cold coagulation syndrome,verify the effectiveness of Jiawei Wutou Decoction in alleviating the pain caused by diabetic peripheral neuropathy,so as to provide more ideas and methods for clinical treatment.MethodsIn this study,60 PDPN patients from the endocrinology department and ward of Tianjin First Affiliated Hospital of Traditional Chinese Medicine who met the criteria for inclusion and discharge were selected and divided into 30 cases in the control group and 30 cases in the test group using a random number table method.The study has passed the ethics Approval and obtain the informed consent of the research subjects.Both groups maintained the original hypoglycemic regimen and the original treatment methods for other diseases.The control group was given oral mecobalamin tablets 0.5mg,3 times a day;the test group was given oral Jiawei Aconitum decoction on the basis of the control group(Zhichuan Wu,white peony,astragalus,ephedra,licorice,cinnamon twig,atractylodes,angelica,dried ginger)200m L,2 times a day,4weeks of treatment.Before treatment,compare the general conditions(gender,age,body mass index,waist-to-hip ratio),course of diabetes,course of PDPN and other diseases of the two groups of patients;observe the visual analogue scale(VAS),toronto clinical system score(TCSS),median and superficial peroneal nerve sensory conduction velocity(SCV),TCM clinical symptom score(total score,main symptom score),blood glucose(fasting blood glucose,blood glucose 2 hours after glucose load),blood pressure(systolic blood pressure,diastolic blood pressure),blood lipids(triglycerides,cholesterol,high-density lipoprotein cholesterol,low-density lipoprotein cholesterol),liver function(alanine aminotransferase,aspartate aminotransferase),kidney function(urea,creatinine)and whether there are adverse reactions during the treatment,SPSS25.0 software is used for statistical analysis of data,so as to evaluate the clinical efficacy of Jiawei Wutou Decoction in the treatment of PDPN with yang deficiency and cold coagulation syndrome.Result1.General situation: The two groups of patients had no statistical difference in gender,age,BMI,WHR,DM course,PDPN course,and past history(P>0.05),and they were comparable.2.Effect on VAS score: Before treatment,there was no statistical difference(P>0.05),and there was comparability.After treatment,there were significant statistical differences within and between the two groups(P<0.01).3.Impact on TCSS score: Before treatment,there was no statistical difference(P>0.05),and there was comparability.After treatment,there were significant statistical differences within the groups(P<0.01),there was no statistical difference between the groups(P>0.05),and there were significant statistical differences in the total effective rate(P<0.01).4.Effect on SCV: Before treatment,there was no statistical difference between the two groups of median and superficial SCV(P>0.05),and there was comparability.After treatment,there was no statistical difference within and between groups(P>0.05),and there was a statistical difference in the total effective rate of SCV in the left median(P<0.05).5.Influence on the total score of TCM clinical symptoms: Before treatment,there is no statistical difference(P>0.05),and there is comparability.After treatment,there were significant statistical differences between the groups(P<0.01),and there were statistical differences between the groups(P<0.05),and there were significant statistical differences in the total effective rate(P<0.01).6.Impact on the scores of main clinical symptoms of traditional Chinese medicine: Before treatment,there was no statistical difference in the scores of other main symptoms except for muscle atrophy(P>0.05),which was comparable.After treatment,there were significant statistical differences in the scores of limb pain and cold limbs between the two groups(P<0.01),and there were statistical differences between the groups(P<0.05);the limbs were soft,numb and soft.There were statistical differences in the asthenia scores within the group(P<0.05),and there was no statistical difference between the groups(P>0.05);only the test group had statistical differences in the hypoesthesia symptom scores compared to before treatment(P<0.05).There was a significant statistical difference in the effective rate only with cold extremities(P<0.01).7.Effects on blood sugar,blood pressure,and blood lipids: there is no statistical difference before treatment(P>0.05),and there is comparability.After treatment,there was a significant statistical difference in 2h PG between the test groups(P<0.01),and there was no statistical difference between the groups(P>0.05).8.Effect on liver and kidney function: Before treatment,there was no statistical difference in liver and kidney function between the two groups(P>0.05),and there was comparability.After treatment,there was no statistical difference within and between groups(P>0.05).9.Adverse reactions: No adverse reactions occurred in the two groups of patients during the treatment.Conclusion1.Jiawei Wutou Decoction can alleviate the pain of PDPN patients with yang deficiency and cold coagulation syndrome,and significantly reduce the VAS score.2.Jiawei Wutou Decoction can improve the clinical symptoms of traditional Chinese medicine in PDPN patients with yang deficiency and cold coagulation syndrome,and has obvious advantages in relieving the pain and coldness of the limbs.3.Jiawei Wutou Decoction has a certain effect on reducing the TCSS score of patients with PDPN syndrome of yang deficiency and cold coagulation syndrome.4.In this study,Jiawei Wutou Decoction did not improve the median and shallow SCV of PDPN patients with yang deficiency and cold coagulation syndrome.5.Modified Aconitum decoction does not affect the blood pressure,blood lipids and liver and kidney functions of patients with PDPN syndrome of yang deficiency and cold coagulation syndrome,and has a certain effect on lowering postprandial blood sugar,and no adverse reactions occurred during the treatment. |
