| Objective: To research the clinical efficacy of liver and spleen concordance method in the treatment of irritable bowel syndrome with diarrhea(liver depression and spleen deficiency syndrome),68 patients with liver depression and spleen deficiency syndrome were observed before and after treatment, and the efficacy and safety of the prescription were objectively evaluated, which has important guiding significance for further clinical promotion of the prescription. Methods: In this study,68 patients with IBS-D(liver depression and spleen deficiency syndrome)were selected from the outpatient department of spleen, stomach, liver and gall diseases of the Affiliated Hospital of Jiangxi University of Traditional Chinese Medicine.Random number table method was used for numbering, among which 34 cases were in treatment group and control group. The treatment group was treated with basic therapy + liver and spleen coordinating prescription, and the control group was treated with Bacillus subtilis and Enterococcus double live bacteria enteric-coated capsules. The course of treatment in both groups was four weeks.Statistical analysis of the changes in TCM syndrome scores, and recurrence rates of the two groups of patients before and after treatment were used to evaluate the curative effect;by observing the vital signs, three routines, liver Renal function test, electrocardiogram and adverse reactions to evaluate the safety of the prescription.Results:(1)Comparison of disease efficacy: i)comprehensive efficacy: the clinical total effective rate of the treatment group was 86.7%,higher than that of the control group(58.1%),and the treatment group was better than the control group(P(27)0.05).Before treatment, there was no significant difference in abdominal pain, stool frequency, stool character, mucinous stool and incomplete feeling of defecation between the two groups(P > 0.05);after treatment, the above symptoms were improved in the two groups(P < 0.01).Compared with the control group, the treatment group was better than the control group in improving abdominal pain, stool frequency, stool character and incomplete feeling of defecation, but there was no significant difference in improving mucus stool between the two groups.(2)Comparison of the efficacy of TCM syndromes: The total clinical effective rate of the treatment group was 93.33%,which was higher than74.19% of the control group, and the treatment group was higher than the control group(P(27)0.05).Comparing the total scores of TCM syndromes, there was no significant difference in the total scores of symptoms between the two groups before treatment(P(29)0.05).After treatment, the total symptom scores of the two groups were significantly reduced(P(27)0.01).Compared with the control group, the total score of the treatment group was lower than that of the control group, and the curative effect was better than that of the control group(P(27)0.01).(3)Comparison of efficacy of individual symptoms: Before treatment, there was no significant difference in the symptoms of abdominal pain and diarrhea, irritability, fullness of the ribs, appetite, anorexia, and fatigue symptoms(P(29)0.05).After treatment, these symptoms of the two groups were improved when comparing within the group(P(27)0.01).Compared with the control group, the curative effect of the treatment group was better than that of the control group in abdominal pain, diarrhea, anorexia, and fatigue(P(27)0.05).In terms of symptoms of irritability and fullness of the ribs, the treatment group was significantly better than the control group(P(27)0.01).(4)There was no significant difference in HAMA and HAMD scores between the two groups before treatment(P ﹤ 0.05).After treatment,HAMA and HAMD scores of the two groups were lower than those before treatment(P(27)0.01).After treatment,HAMA and HAMD scores of the treatment group were lower than those of the control group(P < 0.01).(5)Safety evaluation: During the treatment period of this study,no adverse reactions were found in the treatment group and the control group, and the two groups were highly safe. Conclusion: The method of liver and spleen homology prescription is effective in treating IBS-D(liver depression and spleen deficiency syndrome),which can effectively improve various clinical symptoms, and also has obvious effects on patients’ anxiety, depression, and other mental aspects. No adverse effects were found during the treatment, the safety is high, and the recurrence rate after treatment is low, which is suitable for further clinical research and promotion. |
