| Objective:To explore the relationship between epicardial fat volume and essential hypertension and coronary atherosclerosis.Methods:Patients were randomly selected in the affiliated hospital of Chengde medical college from December,2019 to January,2021,who underwentcardiac examination with SIGNA Pioneer 3.0 T Magnetic Resonance Imaging(MRI)from GE company,According to the inclusion and exclusion criteria,a total of 200 patients were included,including 122 patients with essentialhypertension(EH)and 78 patients with non-hypertension(NH),aged 21~89years,with an average age of 55.83±14.47 years,including 102 males and 98females.According to this,these patients were divided into EH group and NH group.According to the hypertension classification,patients with EH weredivided into three groups:EH1,EH2 and EH3.Sixty-eight of 122 EH patients had undergone coronary CT angiography(CTA).According to the results of CTA,68 cases were divided into Coronary atherosclerosis(CAS)group and Non-coronary atherosclerosis(NCAS)group.Clinical basic data andEpicardial fat volume(EFV)data of each patient were collected.Comparewhether there are differences in clinical basic data between EH group and NH group,whether there are differences in EFV of EH patients with differentgrades,and the correlation between EFV and CAS of EH patients.Results:1.There were significant differences in age,sex,body mass index,TC,TG,HDL-C,LDL-C,smoking and diabetes incidence between EH group and NH group.The EFV in EH group was(130.26±18.85)cm~3,which wassignificantly higher than that in NH group(96.69±9.57)cm~3,with statistical significance.Multivariate Logistic regression analysis showed that there were still significant differences in EFV,TC and body mass index,which were the predictors of essential hypertension.2.The critical value of EFV in predicting EH is 108.49cm~3,the area under ROC curve(AUC)is 0.941(95%CI:0.910-0.971,P<0.05),the sensitivity is87.7%,and the specificity is 88.5%.3.The EFV of EH groups with different grades wasEH3(143.86±15.21)cm~3 higher than that of EH2 patients(136.48±11.51)cm~3and EH1 patients(110.96±10.86)cm~3,respectively.the EFV of EH2 patients was higher than that of EH1 patients,and the difference was statisticallysignificant.4.Compared with EH patients with NCAS,the EFV of CSA patients is larger,and the difference is statistically significant.Multivariate Logistic regression analysis showed that EFV was still an independent risk factor for CAS in EH patients,excluding age,TC and diabetes.Conclusion:The epicardial fat volume is closely related to essential hypertension.The larger the epicardial fat volume,the higher the risk of hypertension,and thehigher the risk of coronary atherosclerosis in patients with essential hypertension. |
