| BackgroundIn recent years,the proportion of two-child nurses in the clinical nursing team has gradually increased,which has affected the orderly progress of clinical nursing work to a certain extent and brought hidden dangers to the quality of nursing.How to reduce or avoid the work-family conflict in nurses with two children,help them smoothly adapt to changes in work and family roles,and then improve their professional recognition,and minimize hidden absences are the key issues that nursing managers need to pay attention to and solve.ObjectiveTo understand the current status of presenteeism of nurses with two children in the third-grade class-A hospitals in Zhengzhou City,analyze the main influencing factors,and provide a theoretical basis for nursing managers to formulate relevant measures to reduce the presenteeism of nurses with two children.MethodsThe nurses with two children in 4 third-grade class-A hospitals in Zhengzhou were selected as the research subjects,the general data scale,Stanford Health Productivity Impairment Scale,nurse pressure source scale and family function scale were used to conduct a questionnaire survey,and the data was statistically processed using SPSS 22.0software.Using single factor analysis(independent sample t test,single factor analysis of variance)、correlation analysis and multifactor analysis(linear regression analysis)to explore the influencing factors of presenteeism.Results1.General information of the nurses with two childrenThe current status of the presenteeism of the nurses with two children 180 questionnaires were issued,174 were recovered,the recovery rate was 96.67%,172 valid questionnaires,and the effective recovery rate was 95.55%.Among 172 nurses,62(36.05%)was under 25 years old when they gave birth to one child,75(43.60%)was25-years old,17(9.88%)was 30-years old,12(6.98%)was 35-years old and 6(3.49%)was more than 40 years old;23(13.37%)was under 25 years old when they gave birth to one child,41(23.84%)was 25-years old,74(43.02%)was 30-years old,22(12.79%)was 35-years old and 12(6.98%)was more than 40 years old.48(27.91%)of the first child’s primary caregivers were parents,87(50.58%)were grandparents or grandparents,37(21.51%)were nannies;36(20.93%)of the second child’s primary caregivers were parents,93(54.07%)were grandparents or grandparents,43(25.00%)were nannies2.The status quo of presenteeism of nurses with two childrenThe average score of 172 nurses with two children for presenteeism was(17.13±2.75)points,which was at a high level.The high presenteeism rate was 55.81%(96/172),and the low presenteeism rate was 44.19%(76/172).3.The factor analysis results of presenteeism(1)Results of single factor analysis : different age,gender,education,working years,monthly income,primary caregivers of one and two children,establishment status,and whether they were shifting work had statistically significant differences in the scores of implicit absenteeism(P<0.05).(2)Results of linear regression analysis:age,gender,marriage,education background,working years,average monthly income,primary caregivers of the first and second children,establishment,shift work or not were predictors of invisible absence.And age 35-years old,female,divorce,junior college,working experience ≤5 years,monthly income ≤5000 yuan,shift work was a risk factor for the presenteeism of the nurses with two children(P<0.05);the primary caregivers of the first and second children were grandparents,grandmothers,babysitters and authorized strength were the protective factors for the presenteeism of the nurses with two children(P<0.05).4.The influence of work stress and family function on presenteeism of nurses with two children(1)The influence of work stress on presenteeism of nurses with two children: The average score of 172 nurses with two children was(82.19±10.47),which was at a high level.The total score of workload and time allocation,working environment and resources,patient care and work pressure of the high presenteeism group was higher than that of the low presenteeism group,and the difference was statistically significant(P<0.05).Pearson’s correlation analysis showed that the score of presenteeism of the nurses with two children was positively correlated with workload and time allocation(r=0.740),working environment and resources(r=0.585),and patient care(r=0.475)(P<0.05);multiple stepwise regression analysis of workload and time allocation,working environment and resources,and patient care were the predictive factors for the hidden absence of the nurses with two children(P<0.05).(2)The influence of family function on presenteeism of nurses with two children The average family function score of 172 nurses with two children was(130.26±21.14),which was in the lower middle level.The scores of RL,BC,GF and total family function of the high presenteeism group were higher than those of the low presenteeism group,and the difference was statistically significant(P<0.05);Pearson correlation analysis showed that the implicit absence of nurses with two children was positively correlated with RL(r=0.459),BC(r=0.579),GF(r=0.558)(P<0.05);in multiple stepwise regression analysis,RL,BC,and GF were the predictive factors for the hidden absence of nurses with two children(P<0.05).Conclusion1.The presenteeism of nurses with two children is at a high level.The independent risk factors are age 35-years old,female,divorce,junior college,working experience ≤5years,monthly income ≤5000 yuan,shift work,and the independent protective factor is that the primary caregivers of the first and second children are grandparents,grandmothers,nanny and authorized strength.This conclusion suggests that nursing managers can take measures to reduce the number of night shift,improve the working treatment and provide a benign competitive working atmosphere to reduce presenteeism level of nurses with two children.2.There is a significant correlation between the presenteeism of nurses with two children,work stress and family function,and among them,workload and time allocation,working environment and resources,patient care,RL,BC,GF have a predictive effect on presenteeism,which are the predictive factors affecting the presenteeism of nurses with two children.This conclusion suggests that nursing managers can reduce the work pressure of nurses and enhance the family function of nurses by adopting flexible scheduling,psychological counseling and family friendly policies,so as to reduce the presenteeism of nurses with two children. |
