Objective To understand the current situation and influencing factors of nurses' moral distress and work alienation in third grade class-A hospital in Shihezi,and analyze the correlations among them.The purpose is to make Medical institutions paid more attention to moral distress and work alienation of clinical nurses which supply the science background dates for essential steps for addressing moral distress and supporting the cultivation of moral resilience in individuals,alleviating the negative experiences nurses,stability nursing human resources,enhancing cooperation between multidisciplinary team members.Methods A self-made demographic data questionnaire,the Chinese version of Moral Distress Scale and the nurse's work alienation Scale was used to investigate a total 767 clinical nurses from two grade-3 class-A hospitals in Shihezi since March to August.Results 1.The total score of nurses' moral distress was (47.41±27.14) and the dimension score was respectively individual responsibility (10.69±3.23),not in patients' best interest (12.44±4.15),value conflict (18.82±5.58),damage patients interest (4.53±1.03).2.There were significant differences on the score of moral distress in nurses with different ages,working years,remuneration,job title and department (P<0.05).3.The total score of nurses' work alienation was (30.18±9.24) and the dimension score was respectively helpless feeling (11.77±3.66),meaningless feeling (9.41±3.665),helplessness feeling (8.99±3.46).4.Moral distress correlates with work alienation in nurses (r=0.457,P<0.01).There was a significant positive relation among dimensions of nurses' moral distress Questionnaire and dimensions of work alienation Questionnaire.5.Multiple linear regression analysis showed that working years,remuneration,job title and department were influencing factors of moral distress,Additionally,the study also found that meaninglessness,sense of powerlessness,remuneration,job title and department could explained 20.8% of the variance for nurse moral distress.Conclusion (1)The moral distress of nurses is generally of low level,working years,remuneration,job title and department were influencing factors of moral distress.Institution leaders need to take essential steps for addressing moral distress and supporting the cultivation of moral resilience in individuals.(2)The work alienation of nurse is generally of moderate level,nursing leaders should pay more attention to nurse's negative emotions,the hospital administrator can carry on in direction mental intervention,improving job satisfaction and organizational commitment level.(3)Moral distress correlates with work alienation in nurses,There was a significant positive relation among dimensions of nurses' moral distress Questionnaire and dimensions of work alienation Questionnaire,Its purpose was to identify strategies that individuals and systems can use to mitigate the detrimental effects of negative emotion and foster moral resilience,Nurses and other members of the health care team need education and mentoring to develop such relational skills as motivation,introspection,empathy,communication,mindfulness,and emotional intelligence in order to exercise moral agency in response to negative emotion.
