| Objective:This study through investigating the needs of home care services for the elderly with chronic diseases based on "Internet +" and analyzing its influencing factors,this paper discusses the willingness of community nurses to choose home care services and its influencing factors,and formulates a home care service plan for the elderly with chronic diseases based on "Internet +" so as to promote the intelligent development of home care services.Methods:1.Analysis on the demand and influencing factors of "Internet +" home care service: This study selected the elderly with chronic diseases in some communities of Sanming City as the research object by random sampling method from June to December,2020.The "Internet +" old-age care service demand scale for the elderly with chronic diseases was adopted(Cronbach’sαtotal coefficient was 0.91,content validity was 0.92).To investigate the general situation of the elderly with chronic diseases and their needs for home care.2.The willingness and influencing factors of "Internet +" community nurses to engage in home care: Registered nurses in some community health service centers in two districts of Sanming were randomly selected to conduct a survey.The survey tools included: general information,questionnaire of community nurses engaged in home care service(S-CVI: 0.88,I-CVI: 0.87-1.0,Cronbach’sa coefficient:0.801).The intention of community nurses to engage in home-based service was investigated.All collected data will be used SPSS24.0 statistical software to establish a database for descriptive statistical analysis,such as percentage,t-test,analysis of variance,and Pearson correlation analysis.3.The cluster sampling method was adopted to select the relevant data of 21 elderly people with chronic diseases from "Kangnursing Online" platform.The general information of the research subjects and the current situation of home care services for elderly people with chronic diseases were investigated.SPSS24.0software was used for data analysis,such as frequency,percentage,standard deviation,analysis of variance,etc.Results:1.Status quot of Demand for Home Nursing Services and Its Influencing Factors(1)The scores of home care service demand for the elderly with chronic diseases in Sanming City were from high to low: Chronic diseases,regular follow-up service(3.84±0.99),regular physical examination service(3.78±0.96),medical diagnosis and treatment and the door to the nearest service(3.77±1.04),drug safety instruction(3.75±1.06),home care services(3.73±1.08),the hotline health knowledge consultation services(3.66±1.06).(2)The requirements of each dimensionThe daily life care needs were low,and the scores were high as follows:medication safety guidance(3.75 ± 1.06),physical work such as installation/maintenance/handling(3.36 ± 1.22),and education of providing safety knowledge(3.35±1.15).The largest demand for medical and nursing services was regular follow-up services for chronic diseases(3.84 ± 0.99),regular physical examination services/free diagnosis and treatment(3.78±0.96),and nearby medical treatment and home diagnosis and treatment services(3.77±1.04).The demand for spiritual and cultural services was moderate,followed by recreational activities(3.54± 1.04),chatting relaxation and psychological consultation(3.50 ± 1.08),rights protection and legal consultation services for the elderly(3.42±1.08).(3)The influencing factors of daily care included age,marital status,family per capita income and self-care ability(P < 0.05).The influencing factors of medical care service included original occupation,marital status and self-care ability(P <0.05).The influencing factors of spiritual and cultural services included average monthly actual income and main economic sources(P < 0.05).Multiple linear regression showed that the demand for home care services included marital status and self-care ability(P < 0.05).2.The influencing factors of community nurses choosing to engage in home care include: personal attitude(development opportunity,obtaining satisfactory salary,higher social status,low work risk,low work pressure),subjective norms(my family),and perceived behavioral control(nursing skills,communication ability,work experience,and national policy).3.The research shows that the older the age,the higher the demand for home care services,mainly medical care services;Correct home care can reduce the complications of the elderly with chronic diseases.52.4% of the elderly with chronic diseases mainly care for their children;Nursing services are expensive and are fully self-paid.Conclusion:1.To provide home health care services for the elderly chronic diseases,to strengthen medical and door-to-door service,to the nearest medical diagnosis and treatment and the door to the nearest service should be strengthened,especially provide convenience for the elderly cannot provide for oneself,make its get timely treatment and help,targeted to meet the elderly chronic diseases individualized demand,make the elderly life quality was improved.2.Give full play to the advantages of community nurses and improve their social status;Set up a professional nursing team,strengthen communication and nursing skills training;To reduce work pressure and risks,improve relevant policies and social support
