Objective To explore the application and effect of "Internet+collaborative nursing model"in the care of patients with chronic wounds in the context of the establishment of county medical community.Methods Sixty-six patients with chronic wounds who were treated first in the wound clinic of the First People's Hospital of xiang shan from April 2019-July 2019 and whose residence was in the branch of the First Medical Health Group were selected.The subjects 66 cases were randomly divided into two groups according to random number table method,33 cases in each group,32 cases in the intervention group and 30 cases in the control group.Both groups used the wet healing method.The intervention group used the "Internet+collaborative care model" to guide the nursing of chronic wounds,while the control group used routine nursing guidance.The duration of intervention was 2 months.Intervention measures:We Chat,cloud clinic,etc.as the "Internet+" carrier,the team intervened in chronic wound patients and caregivers through various methods such as health education,reminders of wound dressing change,and answering wound diseases.Data of wound area,PUSH score,SAS score,social support,and SF-36 quality of life were collected before the intervention,4 weeks after the intervention,and 8 weeks after the intervention.All data were input into the computer,and statistical processing was performed using the SPSS21.0 statistical software package.The counting data were processed and analyzed by ?2-test and described by examples and composition ratio.The measurement data were processed and analyzed by t-test,and the mean(+standard deviation)was described.Independent sample t-test was used to compare the data between groups.Paired sample t-test was used to compare the data within groups.The repeated measurement variance analysis was used to measure the data at different time points within groups.Results ? The baseline of general information,wound type,area size,PUSH score,SAS(anxiety)score,social support and quality of life scores were consistent between the two groups,which was comparable.? The wound healing status of the two groups showed that the wound area and PUSH score of the intervention group were smaller than those of the control group after 4 weeks and 8 weeks of intervention,the differences were statistically significant(P<0.05);The comparison of wound area and PUSH score at different time points in the same group showed that with the prolongation of treatment time,the wound area decreased significantly,and PUSH score decreased significantly(P<0.05);The wound healing rate in the intervention group was significantly higher than that in the control group after 8 weeks of intervention(P<0.05).?The Anxiety of the two groups showed that:After 4 weeks and 8 weeks of intervention,the anxiety score of intervention group was lower than that of control group,and the difference was statistically significant(P<0.05);Anxiety scores at different time points in the same group showed that with the prolongation of treatment time,anxiety scores of patients were significantly improved,with statistical significance(P<0.05).?The social support degree of the two groups showed that:After 4 weeks of intervention and 8 weeks of intervention,especially after 8 weeks,the social support of each dimension in intervention group increased more significantly than that in control group,with statistical significance(P<0.05).The scores of social support dimensions at different time points in the same group showed that with the prolongation of treatment time,the subjective support,objective support and utilization of support of patients were significantly improved,showing that both the control group and the intervention group had certain therapeutic effect,but the improvement effect of social support in the intervention group was better than that in the control group,with statistical significance(P<0.05).?The quality of life of patients with chronic wounds in the two groups showed that after 4 weeks of intervention,except for physiological function(PF),physiological function(RP)and emotional function(RE),the scores of physical pain(SF),energy(VT),mental health(MH),social function(SF),and general health status(GH)in the intervention group were improved compared with those in the control group,and the difference was statistically significant(P<0.05);After 8 weeks of intervention,the scores of each dimension of quality of life in the intervention group were significantly better than those in the control group(P<0.05);With the intervening time going on,the quality of life in the intervention group was significantly improved compared with that before the intervention,and the difference was statistically significant(P<0.05).Conclusion The Internet+collaborative care model provides personalized and targeted health education and nursing intervention for patients with chronic wounds,enabling community and familyization of chronic wound care.Compared with traditional outpatient chronic wound care mode,Internet+collaborative care The model is a chronic wound patient in the county medical community,which can effectively meet the needs of medical care in remote areas and patients with mobility problems.At the same time,it can effectively improve the enthusiasm of medical staff,patients and their families,and ultimately accelerate the healing speed of wounds and reduce anxiety.The appearance of emotions Improve the quality of life of patients,therefore,the Internet+collaborative care model is worth promoting in the county medical community.
