ObjectiveIntegrated psychosocial rehabilitation to improve the quality of life and social function of patients with schizophrenia in the community.MethodPatients with chronic schizophrenia with stable conditions in the community were selected as the research objects and randomized controlled study design was adopted. They were randomly divided into the intervention group and the control group with a 1:1 distribution ratio. The intervention group was given a integrated psychosocial intervention on the basis of conventional treatment such as atypical antipsychotics. The control group only received community care and rehabilitation after taking atypical antipsychotic medication.Integrated psychosocial interventions include work rehabilitation training, social skills training, and family education. These two groups were observed at the beginning of the intervention(baseline)and at the end of the intervention(6 month). The patients’quality of life, social functioning, social skills, family burden and psychiatric symptoms were measured. The scale used includes World Health Organization quality of life-brief(WHOQOL-BREF)、the Personal and Social Performance scale(PSP)、Social Skills Checklist(SSC)、Family Burden Scale of disease(FBS)、Positive and Negative Symptoms Scale(PANSS).SPSS 20.0 software was used for statistical analysis of the data. The primary outcome--the subfractions of each scale between the two groups--were compared using the rank sum test(mann-whitney U).Normal distribution data:The t-test of independent samples was used for the discontinuities of the two groups, and the paired sample t-test was used for comparison before and after the same group.Chi-square test was used to calculate the general data of the two samples of the treatment group and the control group at baseline. The factors affecting quality of life and its improvement were analyzed by multiple linear regression method.Result1、A total of 80 patients were included in the study, including 40 in the intervention group and 40 in the control group. The differences in basic demographic data between the two groups did not reach statistical significance.2、During the study, there were 4 cases dropout in the intervention group(dropout rate:10%),and 3 cases dropout in the control group(dropout rate:7.5%).The difference was not statistically significant(p>0.05).3、After 6 months of observation, the improvement rates of the quality of life scale for the intervention group and the control group were-12.5%vs-1.3%,and the difference was statistically significant(statistical value Z=-6.150,p<0.001);the improvement rates of the PSP for the intervention group and the control group were-12.0%vs-1.9%,and the difference was statistically significant(statistical value Z=-6.831,p<0.001);the reduction rates of SSC for the intervention group and the control group were 22.6%vs 0.00%,and the difference was statistically significant(statistical value Z=-6.124,p<0.001);the reduction rates of FBS for the intervention group and the control group were 11.1%vs 2.8%,and the difference was statistically significant(statistical value Z=-4.289,p<0.001);the reduction rates of PANSS for the intervention group and the control group were 4.20%vs 0.00%,and the difference was statistically significant(statistical value Z=-4.103,p<0.001);Compared with the control group, the intervention group had significantly different score reduction rates on Qo L,PSP,SSC,FBS,and PANSS(p<0.001),which has significance for further promotion and learning.4、After the intervention,y=QOL included x1=SSC(y)and x2=group(y)and x3=FBS(y)and x4=QOL in the baseline(y)(F=441.603,P=0.003,R~2=0.963,adjusted R~2=0.961)by stepwise multiple linear regression.ConclusionConventional community rehabilitation management, such as antipsychotic medication, combined with comprehensive psychosocial rehabilitation interventions can significantly improve the quality of life, social and social functions, psychotic symptoms, and family burden of patients with schizophrenia during the rehabilitation period in the community.Improvements in SSC and FBS predicted improvements in Qol.
