| ObjectiveMusculoskeletal ultrasound was applied to research the efficacy of acupoint embedding or acupuncture therapy on multifidus muscle and the transverse abdominal muscle in sufferers with lumbar disc herniation. The Japanese Orthopaedic Association(JOA)low back pain score was used to analyze the clinical efficacy of relevant treatments, summarize the characteristics of the instructor’s treatment of lumbar disc herniation, and seek one kind of convenient and scientific treatment method for patients with lumbar disc herniation.Methods68 patients with lumbar disc herniated who meet the regulation were randomly divided into acupoint embedding group or acupuncture group by spss25.0 software, and 34 patients in each group.32 cases were included and 2 cases fell off in the acupuncture group,32 cases were included and 2 cases fell off in the acupoint embedding group. The acupuncture group was treated with routine acupuncture, and the acupoint embedding group was treated with acupoint embedding at positive points of Guanyuan,Shenshu and Positive reaction point. The acupuncture group was treated once every other day, three times a week,a total of 4 weeks;the acupoint embedding group was treated once every two weeks,a total of 2 times. The JOA low back pain score and the bilateral thickness of transverse abdominal muscle and multifidus muscle were assessed before and after treatment.Results1.There was no statistically significant difference between the two groups of patients in age, course of disease, and gender(P>0.05);the comparison of JOA low back pain score, transversus abdominis thickness, and multifidus thickness between the two groups before treatment was not statistically significant(P>0.05);Before treatment, the thickness of the nonaffected and affected sides of the transversus abdominis and multifidus muscle in the two groups was compared, and the difference was statistically significant(P<0.01),So the two groups can be compared.2.Japanese Orthopedic Association(JOA)Low Back Pain Score:Intra-group comparison: After treatment, the JOA low back pain scores of the two groups were higher than before(P<0.01).Comparison between groups: There was no statistical difference in JOA low back scores between the two groups after treatment(P>0.05).3.Measurement of bilateral multifissure thickness:Intra-group comparison: the thickness of the non-affected multifidus muscle after treatment in the two groups was thicker than before treatment(P<0.01),and the thickness of the affected multifidus muscle in the two groups after treatment was thicker than before treatment(P<0.01);after treatment,There was a statistically significant difference in muscle thickness between the non-affected side and the affected side in the group(P<0.01).Comparison between groups: After treatment, the thickness of the affected side of the multifidus muscle in the acupuncture group was thinner than that of the acupoint embedding group(P<0.05).There was no significant difference in the thickness of the non-affected side of the multifidus muscle between the two groups after treatment(P>0.05).4.Measurement of bilateral transverse abdominal muscle thickness:Intra-group comparison: the thickness of the non-affected transverse abdominal muscles of the two groups after treatment was thicker than before treatment(P<0.01),and the thickness of the affected transverse abdominal muscles of the two groups after treatment was also thicker than before treatment(P<0.01);after treatment,There was a statistically significant difference in muscle thickness between the non-affected side and the affected side in the group(P<0.01).Comparison between groups: After treatment, the thickness of the transverse abdominal muscles affected in the acupuncture group was thinner than that in the acupoint embedding group(P<0.05).There was no significant difference in the thickness of the non-affected transverse abdominal muscles between the two groups after treatment.(P>0.05).Conclusions1.Acupuncture or acupoint embedding can increase the JOA low back pain score, and improve the symptoms and physical sign of LDH patients.2.After acupuncture or acupoint embedding, local core stable muscle thickness can be increased, which is helpful to increase the stability of lumbar spine and is beneficial to the treatment of lumbar disc herniation.Compared with the acupuncture group, acupoint embedding group can better increase the affected side muscle thickness. |
