Objective:In this study,shear wave ultrasound elastography(SWE)technology was used to measure the stiffness of the bilateral multifidus muscle of patients with single-segment and unilateral herniation of lumbar intervertebral disc herniation;and to measure the bilateral multifidus muscle fat of the herniated segment The degree of infiltration is measured and calculated,and the correlation between the two measured values is studied,and the correlation between the two measured values and the patient's disease course and prominent side is analyzed.Preliminarily explore the application and reference value of real-time shear wave ultrasound elasticity technology in the diagnosis and treatment of patients with lumbar disc herniation.Methods:This study involves patients with unilateral and single-segment lumbar disc herniation who were hospitalized in the Second Department of Cervical,Shoulder,Lumbar and Leg Pain,Sichuan Orthopedic Hospital from February 2020 to February2021.The multifidus muscle stiffness and multifidus fat infiltration of the prominent segment of 58 patients who met the inclusion criteria were measured.According to the elimination criteria and the final screening,the data of 45 patients who meet the requirements were sorted out,statistically analyzed,and conclusions were drawn.Results:After paired T test,the Young's elastic modulus of the affected side was lower than that of the healthy side,and the difference was statistically significant(P<0.05).After paired T test,there was no statistically significant difference in fat infiltration between the affected side and the healthy side(P>0.05).According to Spearman correlation analysis,there is no correlation between the disease course and the Young's modulus of the affected side(rs=0.125,P=0.166),and the disease course has no correlation with the Young's elastic modulus of the uninfected side(rs=0.178,P=0.252).According to Spearman correlation analysis,there is no correlation between the course of disease and the degree of fatty infiltration of the multifidus muscle on the affected side(rs=-0.284,P=0.065),and there is no correlation between the course of disease and the degree of fatty infiltration of the multifidus muscle on the contralateral side(rs=-0.287,P=0.062).According to the Pearson correlation analysis,there was no correlation between the Young's modulus of the patient's affected side and the degree of fat infiltration on the affected side(r=0.169,P=0.267).There was no significant correlation between the Young's modulus of the healthy side and the fat infiltration degree of the healthy side(r=0.019,P=0.902).Conclusion:In patients with lumbar disc herniation,the Young's modulus of multifidus muscle on the contralateral side is higher than that on the affected side.The degree of fatty infiltration of multifidus muscle has no correlation with the side of lumbar disc herniation.In patients with lumbar disc herniation,the elastic modulus of the multifidus muscle on the contralateral side and the affected side has no correlati on with the course of the disease.In patients with lumbar disc herniation,the degree of fat infiltration of the multifidus muscle on the contralateral side and the affected side has no correlation with the course of the disease.There is no correlation between the multifidus muscle elastic modulus and the degree of fat infiltration in patients with lumbar disc herniation.Young's elastic modulus can reflect the functional state of muscles to a certain extent.
