Study On The Impact Of Transtheoretical Model(TTM) Health Education Model On The Quality Of Life Of Breast Cancer Patients

Object: This study aimed to ascertain what an ideal health education programme for patients with breast cancer would be,in terms of facilitating them to make healthy lifestyle changes.The health education programme structure was based on the transtheoretical model of behaviour change and the patients recruited were those who had breast cancer and were undergoing rehabilitation.It was hoped this intervention could then improve both the patients’ physical and mental health,as well as their quality of life.Methods: Participants were selected from a hospital in Xinjiang within a two year period(November 2016-November 2018).130 patients met the inclusion criteria;they were undergoing rehabilitation for breast cancer during this time.A three month randomised-controlled intervention study was undertaken,with 65 patients receiving the intervention and 65 receiving normal care and rehabilitation,which was the control group.Patients in the intervention group were given an individualised plan based on the behavioural stage they were at,and their existing diet and exercise habits.This was communicated with,and they were supported using We Chat and face-to-face coaching.Patients in the control group received health education presentations and were given health education materials.We Chat was used for follow-up in this group.Both groups were evaluated before the intervention and again three months into it,and comparative analysis was used to assess improvement in patient quality of life.Results: 1.Differences in demographic data between the two groups,before the study commenced,including quality of life,health behaviours,behavioural stage,resilience and self-management effectiveness,were not found to be statistically significant(p < 0.05),so the groups were deemed comparable.2.When comparing the before and after data for patients in the intervention group,the quality of life score(93.8 ± 9.0)was higher than that before the intervention(80.2 ± 15.2),and this difference was statistically significant(t =-5.705,p < 0.001).In terms of indicators of behaviour change,levels of resilience,self-management efficacy,amount of weekly exercise,daily vegetable intake,daily fruit intake and behaviour stage all improved after the intervention had occurred it was a statistically significant(p < 0.05)change.However,there was no significant difference in BMI index before and after the intervention(p > 0.05).In the control group,patients’ quality of life,resilience,and self-management efficacy had significantly improved after the intervention(p < 0.05).In contrast,no significant differences were observed in amount of weekly exercise done,daily vegetable or fruit intake,BMI index or behaviour phase(p > 0.05).3.Comparison between the two patient groups showed that the improvement in quality of life score after the intervention was statistically significantly greater(t = 3.076,p < 0.05)in the test group(13.6 ± 18.9)than in the control group(5.4 ± 9.2).Patients in the test group also demonstrated a statistically significant improvement in resilience,selfmanagement efficacy and amount of weekly exercise when compared to the changes in the control group(p < 0.05).Additionally,the change in both daily vegetable and fruit intake after the intervention was statistically significantly higher in the test group(χ2 = 5.575,p < 0.001)and the behaviour change stage progression was more improved in the intervention group than in the control patient cohort,which was also statistically significant(p < 0.05).4.Resilience that the patients with breast cancer exhibited during rehabilitation enabled them to improve their quality of life by way of self-awareness and desire for self-improvement.This accounted for 14.49 % of the total effect ratio.Conclusion: 1.A health education program based on the transtheoretical model of behaviour change has demonstrated it is more effective in improving quality of life,resilience and desire for self-improvement in patients with breast cancer who are undergoing rehabilitation.2.A health education program based on the transtheoretical model of behaviour change is superior to traditional health education.3.A health education program based on this transtheoretical model of behaviour change is better at enabling patients with breast cancer,who are undergoing rehabilitation,to make positive changes to improve their personal health behaviours,which will improve their health overall.
