Evaluation Of The Health-related Quality Of Life Of Spouses Of Breast Cancer Patients And Analysis Of Influencing Factors

Objective:1.To investigate the reliabilit,validity and Sensitivity of EQ-5D-3L in the Health-related quality of life assessment of spouses of breast cancer patients,so as to evaluate the applicability of the scale in measuring the quality of life of spouses of breast cancer patients.2.To explore the influencing factors of health-related quality of life for spouses of breast cancer patients.Method:1.A cross-sectional study was conducted and the spouses of breast cancer patients who received breast cancer treatment in the second department of Breast surgery,the Third Affiliated Hospital of Kunming Medical University(Yunnan Cancer Hospital)from August 2018 to October 2018 were included through face-to-face questionnaire survey.2.Demographic characteristics scale and disease-related questionnaires for breast cancer patients were designed by the researchers,the 12-item health survey questionnaire(SF-12)?the three-level European five-dimensional health status scale(EQ-5D-3L)and Social Support Rate Scale(SSRS)were used.3.The internal consistency reliability Cronbach 'coefficient,criterion-related validity,construct validity and sensitivity were used to evaluate the applicability of EQ-5D-3L scale.4.Univariate and multivariate analyses were performed on the factors affecting the health-related quality of life of the spouses of breast cancer patients.Result:1.The Applicability of EQ-5D-3L1.1 ReliabilityThe calculation result of internal consistency reliability coefficient by Cron bach'? was 0.502.1.2 Validity1.2.1 Criterion-related validityThe health utility score of EQ-5D-3L was moderately correlated with PCS-12(physical component summary,PCS-12)(r=0.46,P=0.0001),and weakly corre lated with MCS-12(mental component summary,MCS-12)(r=0.35,P=0.0001).The score of EQ-VAS was weakly correlated with PCS-12(r=0.37,P=0.0001)and MCS-12(r=0.29,P=0.004).The health utility score of EQ-5D-3L was mod erately correlated with the score of EQ-VAS(r=0.40,P=0.0001).1.2.2 Construct validityTo analyze the convergent validity and discriminant validity.In the analysis of convergent validity,EQ-5D-3L represents the dimension of physiological fun ction has a stronger correlation with PCS-12,EQ-5D-3L represents the dimensio n of psychological function has a stronger correlation with MCS-12;In the anal ysis of discriminant validity,EQ-5D-3L and SF-12 have weak or no correlation in the measurement of different traits.1.3 SensitivityFirst,the EQ-5D-3L scores of 100 survey respondents were grouped accor ding to the median cut-off points of PCS-12 and MCS-12,the score of the E Q-5D-3L health utility score and the EQ-VAS score,that PCS-12 equal to or lo wer than the median array,was lower than PCS-12 higher than the median arr ay;There was no significant difference in the EQ-5D-3L health utility score and EQ-VAS score between MCS-12 equal to or lower than the median array and MCS-12 above the median array(t=-0.9943,P=0.3225).Secondly,the EQ-VAS scores of the 28 responde nts in the "Best health state" in the EQ-5D-3L measurement(all five dimensio ns reported "no problems")were grouped according to the median cut-off point s of PCS-12 and MCS-12,The score of EQ-VAS lower than the median array was lower than PCS-12 equal to or higher than the median array,There was no significant difference in the EQ-VAS score between MCS-12 lower than the median array and MCS-12 greater than or equal to the median array(t=-0.9943,P=0.3225).2.Health-related quality of life and influencing factors of spouse with bre ast cancer2.1 The result of Health related quality of lifeThis study investigated 100 Spouse of breast cancer patients,The health uti lity score of EQ-5D-3L was 0.870,The EQ-VAS score was 78.3.2.2 Factors affecting the Health-related uality of life of Spouse of breast cancer patients2.2.1 Single factor analysisEQ-5D-3L health utility score showed statistically significant differences in breast cancer staging(F=3.24,p=0.03),whether breast cancer could be cured(F=3.96,p=0.02),the knowledge of comprehensive treatment effect(F=9.22,p=0.001),treatment cost and pressure(F=3.93,p=0.02)and social support(F=8.14,p=0.01).EQ-VAS scores were statistically significant in different groups of patients with chronic diseases(F=4.11,p=0.02),spouse breast cancer stage(F=2.75,p=0.047),comprehensive treatment effect of breast cancer(F=6.42,p=0.002)and social support(F=5.33,p=0.02)(p<0.05).2.2.2 Multiple factors analysisAfter controlling for the influencing of other relevant factors,social support and cognition on the treatment effect of breast cancer was the influencing factors of EQ-5D-3L health utility score,which showed that the social support score was the higher,and the health utility score was the higher.Respondents who believed that the treatment was effective had higher EQ-5D-3L health utility score than those who did not know the treatment effect.The family's per capita annual income level and cognition on the treatment effect of breast cancer was the influencing factors of EQ-VAS score.The respondents with the per capita annual income of 50,00-10,000 yuan had a lower EQ-VAS score than the respondents with the per capita annual income?5,000 yuan.The respondents who believed that the treatment was effective had a higher EQ-VAS score than those who did not know the treatment effect.Conclusion:1.The Applicability of EQ-5D-3LEQ-5D-3L was used to measure the quality of life of the spouse of breast cancer patients.In this study,EQ-5D-3L has good reliability and validity,also has good sensitivity in physiology,EQ-5D-3L was considered to have good a pplicability in evaluating the health-related quality of life of spouses in breast cancer patients.2.Health-related quality of life and influencing factors of spouses of breas t cancer patients2.1 Health-related quality of life for spouses of breast cancer patientsThe health utility score of eq-5d-31 was 0.870.Spouses of breast cancer pat ients have poorer health-related quality of life,In terms of health-related quality of life,it is comparable to the disease burden faced by male patients with hy pertension and diabetes.2.2 influencing factorsThe Health-related quality of life of a spouse with breast cancer was influ enced by social support and cognition on the treatment effect.
