Objective:To explore the differences of clinical features and computed tomography(CT)features of bacterial liver abscess in diabetic patients compared with non-diabetic patients.Methods:A total of 59 patients with bacterial liver abscess diagnosed in our hospital from March 2019 to January 2020 were selected,of which 31 cases were included in the diabetic liver abscess group,and 28 cases were included in the non-diabetic liver abscess group.The clinical features and CT manifestations of bacterial liver abscess in the two groups were retrospectively analyzed.The clinical features included general data(sex,age),common clinical symptoms and signs(fever,abdominal pain,chills)and laboratory related examinations(white blood cell count,neutrophil percentage,hypoproteinemia,and septicopyemia).CT features included number of abscesses,size of abscesses,site of abscess,honeycomb sign,cluster sign and abscess ring.SPSS software was used for statistical analysis of the data.The measurement data were in line with normal distribution and homogenous variance,and two independent samples t-test was used.Enumeration data were tested by Chi-square test or Fisher’s exact test.Results:(1)Comparison of clinical characteristics between diabetic group and non-diabetic group:Abdominal pain and sepsis incidence of the two groups were compared,and the difference was statistically significant.In diabetic pyogenic liver abscess group,the incidence of abdominal pain was 32.3%,and that in non-diabetic liver abscess group was 71.4%.The incidence of abdominal pain in diabetic hepatic abscess group was lower than that in non-diabetic liver abscess group(P<0.05).In the diabetic liver abscess group,the incidence of septicopyemia was 25.8%,and that in the non-diabetic liver abscess group was 3.6%.The incidence of septicopyemia in the diabetic liver abscess group was higher than that in the non-diabetic liver abscess group(P<0.05).However,there were no significant differences in gender,age,chills,fever,white blood cell count,neutrophils count and hypoproteinemia between the two groups(all P-values>0.05).(2)Comparison of CT features between diabetic group and non-diabetic group:the number of abscesses,honeycomb sign and abscess ring were compared between the two groups,and the differences were statistically significant.In the diabetes liver abscess group,abscesses imaging characteristics of single,honeycomb sign and no ring of liver abscess were 83.9%,87.1%and 83.9%,respectively;and were 53.6%,46.4%,42.9%,respectively,in the non-diabetic liver abscess group.The incidences of the above abscess characteristics in the diabetic liver abscess group were higher than those in the non-diabetic liver abscess group(all P-values<0.05).However,there were no significant differences in abscess size,abscess site and cluster sign between the two groups(all P-values>0.05).Conclusion:In the diabetes group,abdominal pain cannot be obvious when compared with the non-diabetes group.The incidence of septicopyemia of patients with liver abscess in the diabetic group can be higher.The imaging features are often manifested as more honeycomb sign,no abscess ring,and single lesion,which can be used as imaging indicators to identify diabetes patients with liver abscess.
