The Study On The Medication Rules Of Traditional Chinese Medicine In The Treatment Of Malignant Ascites

Objective To explore the medication rules of traditional Chinese medicine(TCM)in the treatment of malignant ascites(MA),provide medication reference and data support for the TCM treatment of MA,improve the effectiveness and scientificity of clinical treatment of MA,and provide research direction for the clinical research of TCM treatment in MA.Methods Collecting published literatures in the databases of CNKI,Wanfang and VIP,systematically sort out the effective TCM formulations of MA,and statistics the frequency,efficiency,four properties and five flavors,channel tropsim of all drugs,explore the medication rules by association rule analysis and cluster analysis among high-frequency drugs.Results A total of 60 relevant literature and 61 TCM formulations were collected,involving 134 Chinese medicinal medicines,mainly including tonifying deficiency medicines,diuresis and dampness medicines,heat-clearing medicines,qi-regulating medicines,blood-activating and stasis-dispelling medicines,and warm,cold,flat medicines,gan,bitter,pungent medicines were most commonly used.The medicines were mainly classified into spleen,lungstomach,kidney and liver.31 medicines with a frequency of more than 7 were statistically studied,and the core medicines for the treatment of MA were Atractylodes macrocephala,Poria,Polyporus,Alismatis Rhizoma,Astragalus membranaceus,Pericarpium,Glycyrrhiza uralensis.Clustered into 6 groups were ① ginger,jujube,bupleurum,Pinellia,ginseng,licorice,and chaqian;②Alisma orientalis,Polyporus umbellatus,Guizhi,Poria cocos,Zelan;③Chenpi,Dangshen,Longkui,Donggua;④ Scutellaria barbata,Curcuma,Hedyotis diffusa,Astragalus membranaceus,Coix seed;⑤large abdominal skin,Magnolia,Atractylodes,Fructus Aurantii Immaturus,Cyperus,Amomum villosum;⑥Aconite,white peony root,dried ginger,angelica.Frequently-used couplet medicines were Fructus Aurantii Immaturus and Xiangfu,Magnoliae Officinalis and Sharen,Scutellaria barbata and Hedyotis diffusa,Solanum nigrum and Donggua,Chenpi and Cdangshen,Coix Seed and Cheqian,Fuzi and Baishao,Guizhi and Zelan,ginger and Dazao,Ginseng and Banxia,Curcuma and Chaihu,Baizhu and Fuling,Alisma and Zhuling,Astragalus and Dafupi.Conclusion The etiology and pathogenesis of MA is generally deficiency in origin and excess in superficiality,and the main pathological factors are water dampness,followed by cancer toxin,qi stagnation and blood stasis.The treatment is based on treating both symptoms and root causes,strengthening healthy qi and removing pathogenic factors.The main treatments are strengthening healty qi and promoting diuresis,other treatments are clearing heat and detoxicating,promoting blood circulation and removing blood stasis,regulating qi.The basic prescription is Atractylodes macrocephala,Poria,Astragalus membranaceus,Polyporus,Alisma orientalis,Pericarpium,Glycyrrhiza uralensis.According to the clinical manifestations of patients,Xiaochaihu Decoction,Yigong Powder,Wuling Powder,Zhenwu Decoction and Fenxiao Decoction can be used in combination with syndrome differentiation.Drugs can also be used to strengthen the therapeutic effect,such as adding Hedyotis diffusa,Scutellaria barbata to strengthen detoxification and tumor suppression.Zhishi and Xiangfu were added to those with obvious qi stagnation.Wet resistance middle coke plus Magnolia,sand kernel.With diarrhea add coix seed,car front son.Liver depression and blood stasis plus curcuma zedoary,Bupleurum.Deficiency cold vomiting plus ginseng,pinellia,etc.,can be modified according to clinical needs,flexible use,in order to achieve better curative effect.
