Summary Of The Medication Rules Of Gastrointestinal Malignant Tumors And Introduction Of Typical Cases

Objective:To analyze the medication experience of Chief Physician Jiang Chunzhuang in the treatment of digestive tract malignant tumors based on the software of"Traditional Chinese Medicine Inheritance Support System(V2.5)",so as to provide reference for clinical treatment.Methods:The retrospective investigation method was used to collect the medical records of digestive tract malignant tumors diagnosed and treated by Chief Physician Jiang Chunzhuang from September 1,2017 to January 31,2020.The diagnosis and treatment information database was established by using TCMISS.And the medication experience of Chief Physician Jiang Chunzhuang was summarized through descriptive analysis method,association rule analysis method,complex hierarchical entropy clustering analysis method,unsupervised entropy hierarchical clustering and other data mining methods,as well as the introduction of typical medical records.Results:Of the 352 prescriptions for the treatment of digestive tract malignant tumors,a total of 285 drugs were involved,and the top ten drugs in the overall frequency were:Astragalus,Radix Paeoniae Alba,Poria,Dendrobium,Atractylodes,Radix Aucklamdias,Coptis,Smilax glabra,Scrophularia,Pinellia.Drugs of invigorating the spleen and Qi were used most frequently.In terms of the nature,taste and meridian tropism of drugs,the nature and taste was mainly warm,cold,sweet and bitter,and the meridian tropism was mainly spleen,stomach and liver meridians.Common drug pairs,core drug combinations and alternative new prescriptions for treating gastric cancer,liver cancer,intestinal cancer and pancreatic cancer were obtained through data mining methods such as association rule analysis method,complex hierarchical entropy clustering analysis method and unsupervised entropy hierarchical clustering method.Conclusions:1.The commonly used medicines for treating gastric cancer are:Dendrobium-Astragalus Wallberry-Cuttlebone,Pinellia-Huanglian-Astragalus-Poria,etc;the core drug combinations are:Galla Chinensis-Cuttlebone-Bletilla striata-Wallberry-Palm Cinnamon-cornu cervi degelatinatum-Dried Ginger,Astragalus-Pinellia-Coptis-Dried Ginger,etc;The treatment of gastric cancer focused on the use of astringent hemostatic drugs on the basis of warming middle-Jiao and tonifying deficiency,warming and tonifying the spleen and kidney;In the aspect of dispelling pathogenic factors,the method of clearing away heat,detoxifying and drying dampness,promoting blood circulation,removing blood stasis and eliminating phlegm should be used to prevent the heat-clearing drugs from being too bitter and cold and hurting the Yang-qi in spleen.Advisor Jiang was good at warming the Yang-qi in spleen by the interior-warming drugs.2.The most commonly used drug pairs for the treatment of liver cancer are:White Peony Root-Turtle Shell-Poria-Artemisia Capillaris,Prunella vulgaris-White Peony Root-Poria,etc;the core drug combinations are:Hematoxylin-zedoary turmeric-Salvia,White Peony Root-Artemisia Capillaris-Prunella vulgaris、Combined Spicebush-curcumae-Cortex Moutan、Bupleurum-Rhodiola-Turtle shell,etc;The treatment of liver cancer focused on tonifying the spleen and helping digestion for invigorating qi and strengthening spleen,nourishing liver,blood and warming Yang-qi in kidney for regulating and tonifying the liver and kidney,nourishing liver yin and suppressing liver Yang for regulating Yin and Yang;In the aspect of dispelling pathogenic factors,the method of clearing dampness and heat,clearing liver and purging fire,relieving liver and accumulating stasis,regulating qi,promoting blood circulation,removing blood stasis,clearing heat and detoxifying should be used.3.The most commonly used drug pairs for the treatment of intestinal cancer are:Astragalus-Poria-(Woody Atractylodes,Atractylodes),Dendrobium-Astragalus-Poria,etc;the core drug combinations are:Dogwood-Fried Atractylodes-Atractylodes,Phellode-ndron-Herba Cephalanoploris-Swallowwort Rhizome、Psoralea Fruit-Euryale ferox-Alpinia Katsumadai Hayata、Pyrola-Loranthus Parasiticus-Atractylodes,etc;The treatment of intestinal cancer focused on regulating qi and spleen,regulating qi and blood,nourishing yin and promoting body fluid,warming and nourishing kidney yang in the aspect of deficiency in origin;And made good use of the methods of "sthenia syndrome requiring purgation","dampness syndrome requiring expelling","heat syndrome requiring cooling","stagnation syndrome requiring dissipating","retention syndrome requiring attack",the methods of promoting blood circulation,removing blood stasis,eliminating pathogenic factors,clearing heat and toxic materials,inducing diuresis,resolving phlegm,resolving masses,and dredging fu organs also should be used.4.The most commonly used drug pairs for the treatment of pancreatic cancer are:White Peony Root-fried endothelium corneum gigeriae galli-fried millet sprout、Astragalus-Poria、White Peony Root-Radix auckladiae,etc;the core drug combinations are:Radix auckladiae-White Peony Root-Magnolia officinalis,Lignum Sappan-Scutellaria-Prunella vulgaris,Fried Malt-fried millet sprout-fire-dried Fructus Crataeg,Artemisia Capillaris-Turtle Shell-ried millet sprout,etc;The treatment of pancreatic cancer was based on invigorating the spleen,eliminating food,soothing the liver and stomach,meanwhile,invigorating the spleen,regulating qi,clearing heat,detoxifying and dampness,promoting blood circulation,removing blood stasis and unblocking collaterals,and dredging the fu organs,resolving phlegm and resolving stagnation.
