| Objectives: To observe the clinical efficacy of dysphagia after ischemic stroke treated with tongue needling combined with acupuncture on "the Seven Acupoints of the Cranial Base",compare with routine acupuncture to analyze the difference in efficacy,and to explore the advantages of dysphagia after ischemic stroke treated with tongue needling combined with acupuncture on "the Seven Acupoints of the Cranial Base",to provide a new combined acupuncture method for the clinic.Methods: Using the random number table method and random number remainder grouping method,72 patients with dysphagia after ischemic stroke who met the inclusion criteria were divided into two groups.Patients whose random number was even were included in the experimental group and treated with tongue needling combined with acupuncture on "the Seven Acupoints of the Cranial Base",patients whose random number was odd were enrolled into the control group and received routine acupuncture.36 cases in each group were treated for 2 consecutive courses.Kubota swallowing capacity rating method,Toshima Ichiro’s swallowing assessment and Standardized Swallowing Assessment(SSA)were selected as observation indicators of curative effect,Water swallowing test was used as curative effect evaluation standard.Before and at the end of the treatment,the two groups patients were scored for swallowing ability.Finally,the curative effect was evaluated and the data were statistically analyzed.Results: A total of 72 patients were enrolled in this study.During the 2-week study,7patients were dropped and eliminated.Finally,33 patients were included in the experimental group and 32 patients were included in the control group.(1)The comparison between the two groups of patients in terms of gender,age,and course of disease showed that the differences were not statistically significant(P>0.05),and they were comparable.(2)The comparison in the ratings of Water swallowing test,Kubota swallowing capacity rating method,Toshima Ichiro’s swallowing assessment and Standardized Swallowing Assessment(SSA)before treatment between the two groups of patients indicated that the difference was not statistically significant(P>0.05)and was comparable.(3)Comparing between the two groups of patients after treatment and before treatment,the differences in the ratings of Water swallowing test,Kubota swallowing capacity rating method,Toshima Ichiro’s swallowing assessment and Standardized Swallowing Assessment(SSA)were highly statistically significant(P<0.01).(4)Comparing the two groups of patients after treatment,the differences in the ratings of Water swallowing test,Kubota swallowing capacity rating method,Toshima Ichiro’s swallowing assessment and Standardized Swallowing Assessment(SSA)were statistically significant(P<0.05).(5)After 2 courses of treatment,33 patients in the experimental group,8 cases were cured,14 cases were markedly effective,7 cases were effective,4 cases were ineffective,the markedly effective rate was 66.67%,and the total effective rate was 87.88%.32 patients in the control group,3 cases were cured,10 cases were markedly effective,11 cases were effective,8 cases were ineffective,the markedly effective rate was 40.63%,and the total effective rate was 75%.The difference in the markedly effective rate after treatment between the two groups was statistically significant(P<0.05),and the difference in the total effective rate was not statistically significant(P>0.05).Conclusions:(1)Tongue needling combined with acupuncture on "the Seven Acupoints of the Cranial Base" and routine acupuncture are both effective for the treatment of dysphagia after ischemic stroke.(2)Tongue needling combined with acupuncture on "the Seven Acupoints of the Cranial Base" in the treatment of dysphagia after ischemic stroke has a higher the markedly effective rate than routine acupuncture,but the total effective rate of the two is equivalent.(3)The experimental group was treated with tongue needling combined with acupuncture on "the Seven Acupoints of the Cranial Base",after treatment,in the ratings of Kubota swallowing capacity rating method,Toshima Ichiro’s swallowing assessment,Standardized Swallowing Assessment(SSA)and Water swallowing test are more significant than the control group of routine acupuncture.The experimental group can significantly improve the dysphagia after ischemic stroke and is worth popularizing in clinical application. |
