Objective: To evaluate the effect of acupuncture at the acupoint group combined with swallowing comprehensive therapy on swallowing function in patients with dysphagia after stroke using VFSS objective assessment, and to discuss the specific effect of acupuncture at the acupoint group on treatment of this disease.Links and mechanisms to form a more complete theoretical method of diagnosis and treatment programs,in order to expand the application of acupuncture in clinical applications and future research to provide reference materials. Methods: Sixty-two patients who met the criteria were randomly divided into treatment group and control group according to the order of admission. The random number table was randomly assigned to 31 patients in each group. Both groups of patients were given a combination of swallowing therapy(basic drug therapy, rehabilitation training, conventional acupuncture, and psychological counseling),and the treatment group was treated with acupuncture at the acupoint group on the basis of comprehensive swallowing therapy.A total treatment period was 3 weeks. Then use Water drinking test scores and VFSS scales to compare the changes in the scores within and between the two groups.VFSS was used to observe the degree of swallowing disorders and changes in the function of swallowing organs, and to observe the clinical efficacy and safety. Then establish a database, apply SPSS20.0 software to statistical analysis of the integrated data to assess the treatment effect of the two groups. In addition,we should promptly record the adverse reactions occurred in the course of treatment, the safety of acupuncture Evaluation.Result: Two groups of patients in general data(age, disease duration, gender, dysphagia)were no significantly different(P>0.05).1.Clinical curative effecttreatment: Treatment group have 9 patients recovered,12 markedly effective,8 effective,2 ineffective, and the total effective rate reached 93.5%;the control group have 5 patients recovered,8 markedly effective,10 effective,8 ineffective,The total effective rate was 74.2%.After the two groups were compared, the difference was significant(P<0.05);The total effective rate of patients with pharyngeal dysphagia in the two groups was testedand there was significant difference(P<0.05).The total effective rate of patients with oral dysphagia in the two groups was tested and there was no significant difference(P>0.05).2.Water drinking test scores: There was no significant difference in the scores of drinking water between the two groups before treatment(P>0.05);there was a significant difference between the two groups after treatment(P<0.05);the scores of the two groups were compared within each group after treatment, there was a significant difference before treatment(P<0.05).3.VFSS scores: There was no significant difference in the VFSS scores between the two groups before treatment(P>0.05);there was a significant difference between the two groups after treatment(P<0.05);the scores of the two groups were compared within each group after treatment, there was a significant difference before treatment(P<0.05).4.VFSS scores comparison of component: The scores of the oral, pharyngeal, and aspiration levels of the two groups were compared before treatment and there were no significant differences(P>0.05);after the treatment, these three scores of the two groups were significantly different from those before treatment(P<0.05);For oral scores, there was no statistical difference between the two groups after treatment(P>0.05);For pharyngeal and aspiration scores, the difference between the two groups after treatment was significant(P<0.05).After treatment, the swallowing ability of oropharyngeal period in both groups was significantly increased, and the degree of aspiration is significantly reduced. The treatment group was even more effective in improving pharyngeal swallowing function and reducing aspiration. Conclusion: Acupuncture acupoint group(a qiang grous of acupoints acupuncture)combined with swallowing therapy and swallowing comprehensive therapy for patients with dysphagia after stroke can effectively improve the swallowing function of patients and reducing risk of aspiration. The former has a significantly higher therapeutic effect than the latter. The main role of acupuncture in the acupoint group is the pharyngeal phase in swallowing disorders, which can increase the contractility of the pharyngeal muscles and promote the remodeling of the normal swallowing circuit. Therefore,in the course of clinical treatment, this method is applied to patients with pharyngeal dysphagia, targeted therapy, and improve the treatment effect.
