| Objective:To observe the clinical effect of ecthyma in the treatment of dampness-heat disease and explore the characteristics and pharmacological effects of ecthyma.To provide an effective treatment method for difficult-to-treat disease-ecthyma.Method:Ninety patients with ecthyma diagnosed in the Department of Wound Repair and Periodical Vascular Surgery(hereinafter referred to as our department)of Wuhan Hospital of Integrated Traditional Chinese and Western Medicine from July 2019 to October2020 were investigated,and their age,gender and other general conditions were recorded in detail.90 patients were randomly divided into 3 groups:control group,Chinese medicine group and western medicine group,with 30 patients in each group.There was no statistical difference in general data between the three groups(P>0.05).The control group received treatment(disinfection,debridement,bandaging,anti-infection,promoting blood circulation and removing blood stasis,and improving circulation);The Chinese medicine group was added with scorpion ointment for external dressing on the basis of the control group.Western medicine group was treated with platelet-rich fibrin(PRF)on the basis of control group.The treatment course was 4 weeks.The patient was instructed to pay attention to rest,light diet,smoking and alcohol,keep mood as comfortable as possible and avoid strenuous exercise.The total effective rate,cure rate,appearance time of granulation tissue,pain degree,healing time,recurrence rate,size of wound were observed in 3 groups.Statistical analysis was conducted with SPSS software to explore the effect of study on the diagnosis and treatment of ecthyma.Results:(1)Comprehensive efficacy(total effective rate)comparison The total effective rate of the control group was 70%,the total effective rate of the Chinese medicine group was 93.33%,and the total effective rate of the western medicine group was 96.66%.After statistical analysis,the difference was statistically significant(P<0.05).There was no statistical significance between the TCM group and the western medicine group,but there was statistical significance between the TCM group and the control group(P<0.05).(2)Comparison of cure rates The cure rate was 53.33%in the control group,60%in the traditional Chinese medicine group and66.66%in the western medicine group.After statistical analysis,there was no statistical significance(P>0.05).(3)Comparison of appearance time of granulation tissue The appearance time of granulation tissue in the control group was(6.87±0.71)d,the traditional Chinese medicine group was(4.29±0.77)d,and the western medicine group was(5.64±1.15)d.After statistical analysis,the difference was statistically significant(P<0.05).The Chinese medicine group was superior to the western medicine group.(4)Comparison of pain severity Statistical analysis before treatment showed no significant difference in VAS scores among the three groups(P>0.05).VAS scores of the three groups after treatment were significantly lower than before treatment,with significant differences(P<0.01);After treatment,there were significant differences in VAS scores among the three groups.According to statistical analysis,there was no significant difference in VAS scores between the traditional Chinese medicine group and the western medicine group,but the difference was significant compared with the control group(P<0.01).(5)Comparison of healing time The healing time was(21.44±2.34)d in the control group,(15.57±1.07)d in the traditional Chinese medicine group and(15.14±1.44)d in the western medicine group.After statistical analysis,the healing time of the three groups was significantly different(P<0.01),and the western medicine group was better than the traditional Chinese medicine group than the control group.(6)Comparison of recurrence rates.Patients were followed up for 6 months.In the observation group,7 cases had recurrent ulcers,and the recurrence rate was 43.75%,including 5 cases of recurrent ulcer in situ and 2 cases of new ulcer.In the TCM group,3 patients had recurrent ulcer,and the recurrence rate was 16.66%,including2 patients with recurrent ulcer in situ and 1 patient with new ulcer.In the western medicine group,1 patient had recurrent ulcer,and the recurrence rate was 5%,including 0 patients with recurrent ulcer in situ and 1 patient with new ulcer.According to statistical analysis,the recurrence rate of the three groups was significantly different,among which,the recurrence rate of the western medicine group was statistically significant compared with the observation group(P<0.05).(7)Wound area comparison The wound area(mm~2)of the observation group before and after treatment was(592.50±66.03/255.83±42.89);The wound area(mm~2)of TCM group was(579.97±76.63/159.17±40.73);The wound area(mm~2)of the western medicine group was(584.17±54.73/196.67±58.44);Before treatment,statistical analysis showed that there was no significant difference in wound area between the three groups(P>0.05).After treatment,the wound area of the three groups was significantly smaller than that before treatment,and the difference was significant(P<0.01).After treatment,there was a significant difference in the wound area between the three groups,in which there was no significant difference between the traditional Chinese medicine group and the western medicine group,but there was a significant difference between the three groups and the control group(P<0.01).(8)Comparison of wound exudation scores Before treatment,there was no significant difference in the exudation score among the three groups(P>0.05).The exudate scores of the three groups after treatment were all lower than that before treatment,with statistical significance(P<0.05).The exudation scores of patients in the TCM group and the western medicine group were lower than those in the control group,with statistical significance(P<0.05).Conclusion:Application rich platelet fibrin,scorpion treat ecthyma disease syndrome,can effectively improve the total effective rate and healing rate,reduce the degree of pain,shorten the healing time,reduce the recurrence rate,reduce the wound area,curative effect is distinct,worthy of clinical application,the scorpion extract curative effect is better,in terms of go saprophytic muscle pain PRF is better in terms of wound repair. |
