| Objective:Through the study of ancient literature and modern treatment experience,the practical and efficient ankle fracture reconstruction techniques were summarized,and a new type of over-ankle splint designed by a new concept was adopted.To confirm the clinical effect of the combined treatment of reduction manipulation and new splint external fixation(referred to as two-in-one characteristic treatment)in the perioperative period of ankle fracture,the researcher can objectively evaluate its effectiveness in accelerating swelling reduction and reducing pain.Methods:(1)Through the study of ancient literature and modern treatment experience,this paper summarizes a set of optimal manipulative methods for ankle fracture.(2)To study the swelling and appearance of the affected limbs in patients with stable ankle fracture,and to create a unique new type of over-ankle splint which fits the swelling shape after stable ankle fracture.(3)From August 2018 to August 2019,60 patients with ankle fracture were randomly selected from the Department of Orthopaedics,affiliated Hospital of Shandong University of traditional Chinese Medicine,and randomly divided into treatment group(n=30)and control group(n=30).The patients in the treatment group were treated with manual reduction and new type over-ankle splint fixation before operation,and the patients in the control group were treated with manual reduction and plaster external fixation before operation.The swelling grade of affected limb,pain score,hospitalization cost and time and AOFAS score were observed and counted before treatment,on the 1st,3rd and 7th day of treatment.The data were analyzed and processed by SPSS22.0 software,the differences between the two groups were analyzed and compared within the group,and the therapeutic effect was evaluated.Results:(1)A set of simple,efficient and standard optimal manual reduction methods are summarized.(2)To make a unique new type of over-ankle splint which fits the swelling shape after stable fracture of ankle joint.(3)By comparing the swelling grades of the two groups,there was significant difference on the 3rd,5th and 7th day after treatment(P<0.05).Compared with that before treatment,the treatment group began to show statistical difference on the 5th day(P<0.05),and there was significant difference between the two groups on the 7th day(P<0.05).(4)There were significant differences in the degree of pain between the two groups on the 1st,3rd,5th and 7th day after treatment(P<0.05).After comparison with that before treatment,there was a significant difference in the treatment group on the 3rd day(P<0.05),and on the 5th and 7th day after treatment,there was significant difference between the two groups(P<0.05).(5)Through the comparison of hospitalization time and hospitalization expenses between the two groups,there was a statistical difference(P<0.05).(6)There was no significant difference in postoperative AOFAS score between the two groups(P>0.05).Conclusions:The summary of plastic manipulation combined with the two-in-one characteristics of new over-ankle splint external fixation in the treatment of ankle fracture in perioperative period has some advantages in reducing swelling and pain,shortening hospitalization cost and time,etc.The therapeutic effect of the new type of over-ankle splint is better than that of traditional plaster external fixation. |
