Objective:This article retrospectively analyzes the therapeutic effect of manipulative splint fixation and external fixation for the treatment of distal radius fracture and provides clinical data and references for clinicians for the treatment of distal radius fracture.Method:According to the inclusion criteria and exclusion criteria,68 patients with distal radius fracture of type B2,B3 and C1 were selected and divided into the manual splinting fixation group and the external fixation group based on different treatment methods and 34 cases in each group.All those patients were from Hubei Provincial Hospital of TCM from 2016.1 to 2017.1.All of them are treated with functional rehabilitation exercises and took Chinese dialectical medicines under the guidance of doctors after surgery.To collecte the wrist radiographs from all patients before treatment one day and one day,one week,three weeks,six weeks,6 months after treatment.To measure the changes of radial height,palmar tilt angle,ulnar deviation and to observe the clinical healing time of fractures.To record the scores of Gartland-Werley wrist function assessments at 6 weeks and 6 months postoperatively.To summarize all clinical data and to establish a database,then to process the collected data by using SPSS 19.0 statistical software and to make relevant results.Results: All 68 patients were followed up effectively for 6 months.One patient's fingers become numbness and dark purple color after 3 days in the manual splint fixation group.The splint tightness was adjusted in time and returned to normal.No adverse reactions and complications occurred in the other patients.The specific results are as follows: 1.After chi-square test analysis,the two treatment groups in terms of age,gender,type of fracture,injury side,injury way,P>0.05,indicated that there was no significant difference in the general data composition between the two groups;The comparison of the radial height,palmar tilt angle,ulnar deviation between the two groups,P>0.05,showed that there was no significant difference between the two groups in terms of imaging before treatment.The results showed that the two treatment groups were from a unified whole and comparable.2.Comparison between the two groups in clinical healing time of fractures,P>0.05,showed no difference in the clinical healing time of fracture.3.In aspects of radial height,palmar tilt angle and ulnar deviation,there was significant difference between one day after treatment and before treatment in the groups,P<0.01,indicating that both treatment methods can be effective for distal radius fracture;Comparison between groups and within groups,all obtained P>0.05,indicating that there was no difference in the radial height,palmar tilt angle,ulnar deviation between the two treatment groups in each period postoperatively.Two methods can maintain the stability after fracture reduction;4.In term of Gartland-Werley wrist function score rate,the difference was statistically significant about the comparison between the two groups after 6 weeks,P<0.05,indicating that the manual splint fixation group was superior to the external fixations group in the early functional score of the wrist joint.Compared with the two treatment groups at the 6th month,P>0.05,there was no statistical difference,indicating that there was no significant difference in the excellent rate of wrist joint function scores at the 6th month.Conclusion: In the clinical healing time of fracture,there was no significant difference between manual splint fixation and external fixator fixation.In terms of radial height,palmar tilt angle,ulnar deviation recovery,both treatment methods had significant reduction effect and stability were maintained during fixation.Early manual manipulation of splint fixation was superior to external fixator fixation in wrist function,but ultimately there was no significant difference in wrist function recovery between the two groups.Therefore,manipulating the splint fixation and the external fixator fixation are effective methods for the treatment of distal radius fracture B2,B3,and C1,and can be widely used in clinical practice.
