| ObjectiveTo observe the clinical effect of reinforcing-reducing manipulations by lifting and thrusting the needle on Upper Limb spastic Paralysis after Stroke,and provide clinical ideas for acupuncture on this disease.Method70 inpatients meeting conclusion criteria with Upper Limb spastic Paralysis after Stroke were selected,which came from Acupuncture Rehabilitation Department in GuangDong Second traditional Chinese Medicine Hospital,from March 2019 to December 2019.Patients were randomly divided into treatment group and control group at a ratio of 1:1,with 35 cases in each group.The treatment group was treated with reinforcing-reducing manipulations by lifting and thrusting the needle.The control group was treated with ordinary acupuncture.Acupoints:Jiquan(Ht1),Quze(PC3),Neiguan(PC6),Jianyu(LI15),Tia njing(SJ10),Waiguan(SJ5).Both groups were treated once a day for 6 days per week,for a total of 2 weeks.Efficacy evaluation indicators:modified Ashworth activity scale(MAS),upper limb simplified Fugl-Meyer Assessment(FMA),and activity of daily living rating scale(ADL,Barthel Index).Patients were evaluated before and after the treatment.All data were analyzed by SPSS21.0 Measurement datas were firstly test for normality.T-test was for the fits one while Mann-Whitney U test was for the no-fits one and rank datas.Numeration dates were subjected to χ2 test.ResultA total of 70 cases were collected in this trial.During the whole test and treatment process,3 people failed to complete the treatment due to transfer hospital to another or voluntary abandonment,while 2 cases in treatment group,1 case in control group.In the end,67 patients completed the clinical trial.there were 33 in the treatment group and 34 in the control group.there was no significant difference in age,gender,course of disease,and type of stroke between the two groups(P>0.05),revealing the comparability between the two groups.Before the treatment,there was no significant difference in the modified Ashworth activity scale(MAS),upper limb simplified Fugl-Meyer Assessment(FMA),and activity of daily living rating scale(ADL,Bar thel Index)between the two groups(P>0.05).It showed the comparability in the two groups.Compared with before and after the treatment for two weeks,there were significant differences in the modified Ashworth activity scale(MAS),upper limb simplified Fugl-Meyer Assessment(FMA),and activity of daily living rating scale(ADL,Barthel Index)between the two groups(P<0.05).After the treatment,there were significant differences in the modified Ashworth activity scale(MAS),upper limb simplified Fugl-Meyer Assessment(FMA),and activity of daily living rating scale(ADL,Barthel Index)in the two groups(P<0.05).There was a significant difference in the efficacy classification between the two groups(P<0.05).The total efficiency was90.91%and 69.70%in the treatment group and the control group.The total efficiency was significanty different in the two groups(P<0.05).Conclusion1.Ordinary acupuncturere and inforcing-reducing manipulations by lifting and thrusting the needle were both effective in treating Upper Limb spastic Paralysis after Stroke.2.Inforcing-reducing manipulations by lifting and thrusting the needle was superior to the simple acupuncture on relieving upper limb spasm and improving athletic ability of upper climb and activities of daily living. |
