ObjectiveTo explore the clinical effect of the Bone Injury Rehabilitation External Washing Granules(BIREWG)on the early postoperation of Terrible Traid Injury of the Elbow(TTIE).Methods70 patients with TTIE were randomly divided into two groups:control group and observation group.The patients in the two groups were treated with the same operation method,and the patients in the control group were treated with simple functional exercise rehabilitation,while the patients in the observation group were treated with fumigation and washing of the BIREWG on the basis of functional exercise at 2 weeks after operation.At 8 weeks after operation,the difference between the two groups in VAS pain score(Visual Analogue Score of pain),the range of elbown motion,DBYA score(De Boar YA function score of elbow joint),DASH score(Disability of Arm,Shoulder and Hand),excellent and good rate of elbow and incidence of common complications were observed.Results62 patients were finally followed up in the study,the lost visit rate was 11.4%,30 cases in the control group and 32 cases in the observat ion group.There was no statistical difference in the general conditions of the two groups,including age,gender,injured side,time from injury to operation,operation time,intraoperat ive bleeding volume,intraoperative tourniquet use time and fracture classification(P>0.05).There was no significant difference in VAS pain score,the range of elbow flexion,extension and rotation,DBYA score and DASH score between the two groups 2 weeks after operation(P>0.05).Pain score:8 weeks after operation,there was no statistical significance in VAS pain score and pain score of DBYA between the two groups(P<0.05).Comparing the difference value between 2 and 8 weeks after operation in VAS pain score and pain score of DBYA,there was no statistical significance(P<0.05).Elbow function and activity:8 weeks after operation,there were statistically significant differences in elbow flexion and extension range,rotation range,daily life score of DBYA and movement score of DBYA between the two groups(P<0.05).Comparing the difference value between 2 and 8 weeks after operation in elbow flexion and extension range,rotation range daily life score of DBYA and movement score of DBYA,there was no statistical significance(P<0.05).The overall curative effect of the two groups:8 weeks after operation,there was a statistical difference in the total score of DBYA and DASH score between the two groups(P<0.05).Comparing the difference value between 2 and 8 weeks after operation in the total score of DBYA and DASH score,there was a significant difference(P<0.01).According to the results of DBYA’s total score,there were 4 excellent cases,11 good cases,14 acceptable cases and 1 bad case in the control group,the excellent and good rate was 50.0%;while there were 11 excellent.cases,15 good cases,acceptable cases and 1 bad case in the observation group,the excellent and good rate was 81.3%.The difference in the overall effect distribution and the excellent and good rate between the two groups was statistically significant(P<0.05).Complications:8 weeks after operation,10 cases of heterotopic ossification occurred in the control group,12 cases of elbow stiffness,the rate of heterotopic ossification was 33.3%,the rate of elbow stiffness was 40.0%;9 cases of heterotopic ossification occurred in the observation group,5 cases of elbow stiffness,the rate of heterotopic ossification was 28.1%,the rate of elbow stiffness was 15.6%.There was no significant difference between the two groups in the rate of heterotopic ossification in 8 weeks after operation(P>0.05),but.the difference in the incidence of elbow stiffness was statistically si gnificant(P<0.05).ConclusionEarly fumigation and washing of the BIREWG can not effectively reduce the pain and the incidence of heterotopic ossification,but it is beneficial to increase the functional range of elbow joint,reduce the incidence of elbow joint stiffness,prevent the occurrence of elbow joint dysfunction,shorten the rehabilitation process,and improve the excellent rate of patients as well as the quality of life.
