Objective:To evaluate open reduction with T type locking compression plate internal fixation and limited open reduction and external fixation combined with Kirschner wire fixation of the two operation schemes in treatment of unstable distal radius fractures, study of unstable distal radius fractures treatment options.Methods:From September 2012 to September 2014 in Affiliated Hospital of Xinjiang Medical University Fifth Department of orthopedics income hospital with unstable distal radius fracture surgery patients as the research object, according to the diagnostic criteria, inclusion criteria and exclusion criteria selected to meet the requirements of a total of 51 patients were 51 sides. There were 24 males,27 females, average age is 56 years old. Left 20, on the right side of 31. According to AO classification, type B 28 cases, type C 23 side. Open reduction and internal fixation group of 26 cases, limited open reduction and external fixation combined with Kirschner wire fixation group of 25 cases. Record the operation time, postoperative 3 month,6 months follow-up measurement of wrist motion (flexion, dorsiflexion, ulnar deviation, radial deviation) data and related imaging (palmar tilt, ulnar deviation, radial shortening, articular surface recovery), according to Gartland-Werley functional evaluation of patients with carpal joint function, carried on the analysis to the two sets of data.Results:ORIF group of average operation time of about 76 minutes, IFEF group of about 55 minutes. In 51 patients, all were followed up, after 6 ~ 12 months of clinical follow-up observation of all patients achieved bone union. The ORIF group had incision infection in 1 cases; group IFEF had pin tract infection in 5 cases. During the follow-up period compared to ORIF group of wrist motion, and imaging data no significant difference with IFEF group, the P value was greater than 0.05, no statistical significance. According to the Gartland-Werley functional assessment evaluation, ORIF group: excellent in 14 cases, good in 10 cases,2 cases, the excellent and good rate was 92.31%; IFEF group:excellent in 8 cases, good in 15 cases,2 cases, the excellent and good rate was 92%, two groups of patients obtained satisfactory wrist function recovery, no obvious difference between two groups rate, P> 0.05, no significant difference.Conclusion:Open reduction with T type locking compression plate internal fixation and limited open reduction and external fixation combined with Kirschner wire fixation of the two operation schemes in treatment of unstable fractures of the distal radius, wrist joint function recovery without significant difference.
