Study Of Disease Spectrum Of Emergency Department Attendants From 2011 To 2012 In Tianjin Medical University General Hospital

Objective:To analyse the pattern of disease spectrum in emergency department of Tianjin Medical University General hospital.To analyse the regularity and problem of patients’visiting doctors.To study the regularity of special disease,provide evidence for the organization of hospital,cultivation of emergency talents and management of medical resource.Methods:Choose the patients to study who visited doctors in Tianjin Medical University General hospital emergency department from 1st July,2011 to 30^thJune,2012.Collect the name、gender、time、diagnose by management system in hospital.Count and analyse the data by Excel 2007 software.List chart to research the regularity of disease spectrum in emergency department.Result:（1）145413 cases are included in the stactics.The average number of cases ereryday is 398.4.The number of female patients exceeds males.According to the number of cases,the top three departments are internal medical department（42.8%）、fever clinic（11.6%）、neurology department（8.7%）.（2）According to the number of cases,the laregest months are April、June、Augest,the minimum month is February.The time of visiting doctors is regularly.From 2:00 to 6:00,the number of patients is the least,from 8:00 to 10:00,the number is the second largest,from 17:00to 23:00,the number is the largest.（3）In the rankings of diseases’amount,from first to thirtieth,the sum is 122880,84.1%of the total number.The top five in emergency department are upper respiratory tract infection、injury、gastroenteritis、fever、abdominal pain among all the diseases according to the number of cases.In the internal medical diseases,the top three are respiratory system、circulatory system、alimentary system according to the number of cases.The number of check or taking the medicine ranked the 13^th in all the diseases and ranked the 4^th in the internal medical diseases.It pointed out that some non emergency patients visited doctors in emergency department who occupied the resource of emergency.（4）According to the sex、age and season,different diseases have their special patterns.Concerning upper respiratory tract infection,the proportion of females（59.5%）exceeds males（40.5%）.The peak age is 20～29.The peak month is January,the nadir month is November.Concerning injury,the proportion of males（63.5%）exceeds females（36.5%）.The peak age is 20～29.The peak months are May and June,the nadir month is February.Concerning gastroenteritis,the proportion of females（59.7%）exceeds males（40.3%）.The peak age is 20～29.The peak months are June to September,the nadir month is February.Concerning pneumonia/bronchitis/COPD,the proportion of females（54.3%）exceeds males（44.7%）.The peak age is over 80.The peak month is January,the nadir month is July.Concerning heart failure,the proportion of females（52.7%）exceeds males（47.3%）.The peak age is over 80.Concerning AMI,the proportion of males（52.6%）exceeds females（47.4%）.The peak age is over60～69.Concerning hematencephalon,the proportion of males（65.8%）exceeds females（34.2%）.The peak age is 50～59.Concerning ACI/ischemic cerebrovascular disease,the proportion of males（52.9%）exceeds females（47.1%）.The peak age is60～69.Conclusion:The research reveals the composition of emergency disease spectrum and pattern of special diseases.According to the disease spectrum,we can improve the management of medical resource,arrange an order to doctors、nurses、pharmacy staff、laboratory staff and radiology staff.So we can improve medical quality.The disease spectrum provides evidence for development of emergency personnel and member exchange.According to the problem mentioned by disease spectrum,it is necessary to make a emergency criteria by the government.
