Objective: This study aimed to analyze the correlation between different clinical characteristics of colorectal polyps and Helicobacter pylori(Hp)according to histopathological type,severity,anatomical site and the number of lesions.Methods: The study participants were patients who underwent colonoscopy in the Department of Gastroenterology,Zhongda Hospital affiliated to Southeast University from January 2016 to December 2018.A total of 409 patients with adenomas,207 patients with serrated polyps,and 507 patients as polyp-free control group met the inclusion and exclusion criteria.The diagnosis of positive Hp infection was based on three detection methods: urea breath test,rapid urease test and histological examination of biopsy gastric specimens.We compared the rate of Hp infection between case groups and the polyp-free control group,and used multivariate logistic regression to analyze the correlation between colorectal polyps and Hp infection.Results:1.Based on histopathology,Hp infection was independently associated with colorectal adenomas(OR=1.953,95%CI:1.467-2.601),and Hp infection was independently associated with serrated polyps(OR=2.812,95%CI:1.953-4.047)compared with the control group.2.Based on lesion severity,Hp infection significantly increased the risk of non-advanced adenomas(OR=2.388,95%CI:1.523-3.743)and advanced adenomas(OR=1.792,95%CI:1.305-2.460).Hp infection significantly increased the risk of non-advanced serrated polyps and advanced serrated polyps(OR=2.446,95%CI:1.656-3.611;OR=5.528,95%CI:2.400-12.733).3.Based on anatomical site,the positive association of Hp infection was observed in both proximal colorectal adenomas and distal colorectal adenomas(OR=1.860,95%CI:1.241-2.790;OR=2.129,95%CI:1.479-3.065),and it was also observed in both proximal serrated polyps and distal serrated polyps(OR=2.390,95%CI:1.375-4.154;OR=2.772,95%CI:1.775-4.327).4.Based on the number of lesions,Hp infection was associated with a higher risk of single adenoma(OR=2.032,95%CI:1.479-2.793)and multiple adenomas(OR=1.750,95%CI:1.135-2.700).Hp infection was associated with a higher risk of single serrated polyp and multiple serrated polyps(OR=2.839,95%CI:1.881-4.283;OR=2.873,95%CI:1.539-5.364).Conclusion: Hp infection was positively correlated with colorectal polyps of different histopathological types,severity,anatomical site and the number of lesions.
