OBJECTIVE: This study explored the relationship between Helicobacter pylori(HP)infection and different pathological types of colorectal polyps(CP)and colorectal cancer(CRC).This study also analyzed HP’s infection rate in each parameter of Adenomatous polyps(AP).The purpose of this study is to provide certain clinical evidence for HP eradication indications and colorectal tumor screening. Methods: 1.A total of 518 patients who attended the Department of Gastroenterology and Oncology of the First Affiliated Hospital of Hebei North University from January 2019 to January 2021 were selected. All patients underwent electronic gastroscopy, electronic fiber colonoscopy and 13 C urea breath test(Urea Breath Test,UBT)examination.According to the inclusion and exclusion criteria, all patients were divided into 126 cases in the Inflammatory polyps(IP)group,118 cases in the Hyperplastic polyps(PP)group,108 cases in the AP group,74 cases in the newly treated CRC group and 92 cases in the non-polyp control group.2.Collect the gender, age and the number of positive HP infections in each group, and collect the DOB value of each pathological type in the AP group determined by UBT.Statistical analysis was performed on the obtained data to evaluate the differences in the HP positive rate among the groups.3.This study further analyzed the correlation between the possibility of canceration of each pathological type of AP and the semi-quantitative index of HP colonization density in the stomach. Results: There was no statistically significant difference in the HP positive rate between the IP group and the PP group compared with the control group without polyps(P>0.05).Compared with the control group, the positive rates of HP in the AP group and CRC group were significantly different(P<0.05).It is suggested that AP and CRC are related to HP infection. There was no significant difference in the HP positive rate between the AP group and the CRC group(P>0.05).Further comparison of the HP positive rate of each subgroup in the AP group showed that the HP positive rate of the polyp diameter ≥1.0cm group was significantly higher than that of the polyp diameter <1.0cm group, and the HP positive rate of the polyp multiple group was significantly higher than that of the polyp single group. The difference in HP infection rate between the two parameter groups was statistically significant(P<0.05).It is suggested that the size and number of AP are related to HP infection.Regarding the location of the largest polyp in a case, this study divides AP into three groups. The first group contained the ileocecal area and the ascending colon. The second group included liver area, transverse colon and spleen area. The third group was descending colon, sigmoid colon and rectum. The difference between the three groups was not statistically significant(P>0.05).According to the condition of the polyp pedicle, there was no statistically significant difference in HP positive rate between the pedicle group and the non-pedicle group(P>0.05).According to the AP pathological type, there was no significant difference in the HP positive rate between the tubular adenoma group, the tubular villous adenoma group and the villous adenoma group(P>0.05).There was statistically significant difference among the three groups of DOB value measured by UBT(P<0.05).The semi-quantitative index DOB value of HP colonization density is positively correlated with the possibility of canceration of AP pathological type.Conclusion:1.HP infection is related to colorectal AP and CRC,but has nothing to do with IP and PP.2.HP infection is related to the size and quantity of AP,and has nothing to do with the location and pedicle.3.The colonization density of HP in the stomach is related to the pathological type of AP.The more likely the pathological type is to become cancerous, the higher the colonization density of HP in the stomach.
